Job summary

This is a hugely exciting opportunity to work with a range of  organisations to enhance the experience of patients within the Wythenshawe area and work collaboratively with our new clinical team to help design and refine the way we improve care in our Care Homes.

The successful candidate will have great relationship and communication skills and will have an understanding of primary care services & community health services.

Main duties of the job

The post holder will work across the seven Wythenshawe PCN GP practices (Benchill Medical Practice, Bowland Road Medical Practice, Cornishway Group Practice, Peel Hall Practice, Maples Medical Centre, RK Medical Practice, Tregenna Group Practice) and will be required to travel from practice to practice and to care homes within the PCN area during fulfilment of their duties, therefore a full clean driving license is essential, plus access to a reliable vehicle.

About Wythenshawe Primary Care Network Enhanced Health in Care Homes (EHCH) Service

The Wythenshawe EHCH service is a brand new service established to deliver the Enhanced Care in Homes framework. We support the patients registered across our seven GP practices living in CQC registered care homes

People living in care homes should expect the same level of support as if they were living in their own home. This can only be achieved through collaborative working between health, social care, Voluntary, Community, and Social Enterprise (VCSE) sector and care home partners. The Enhanced Health in Care Homes (EHCH) model moves away from traditional reactive models of care delivery towards proactive care that is centred on the needs of individual residents, their families and care home staff. Such care can only be achieved through a whole-system, collaborative approach.

Job description

Job responsibilities

Overall responsibility for arranging the monthly Care Home MDT meetings and the smooth running of integrated care within the PCN. The key role of the Care Coordinator will be to schedule the MDT meetings, manage the meeting agenda items, write up minutes and ensure that actions are recorded and completed.

Liaise with all clinical and non-clinical members in the MDT to ensure effective MDT function.

Support the Clinical Lead for this work stream through providing administrative support, where required.

Manage reporting required and associated within the DES specifications for required services.

The care-coordinator would deal with the daily queries received from the care homes, seeing the queries through to completion and linking in with the relevant clinicians to enable this completion which will include the GPs, ANPs and clinical pharmacists.

Support the development of a PCN-wide Winter Pressures Plan for our care homes, supporting the rollout of Flu Vaccinations and blood tests.

Signpost practice team members, service users and carers to relevant services including the Social Prescribing Link Worker Service.

Liaise with other stakeholders as needed for the collective benefit of patients including but not limited to Patients, GPs, Nurses, other practice staff and other healthcare professionals including pharmacists and pharmacy technicians from provider and commissioning organisations.

Develop excellent working relationships with the all partners, wider service networks including the voluntary sector, GP practices, adult social care, hospitals, community pharmacists and other members of the MDT

Providing a point of contact for both staff and patients in coordinating care and will help to deal with incoming queries.

Support patients to access personalised care and support plans, in line with best practice.

Ensure regular and consistent communication with care homes regarding patient progress and any complications, including supporting the coordination of ward rounds

Provide these cohorts of people signposting to identified services in order to maintain their independence and improve their health and well being
