ABOUT THE ROLE

This role will focus on supporting and empowering individuals to maintain their mental health, also in supporting them to be part of the wider community to help them lead more fulfilling lives. The role will provide pathways, support and appropriate interventions to enable successful resettlements. This role will ensure individuals who are receiving support or require this support, have meaningful conversations about their housing and social care needs.

This role will work alongside the NHS through the Long Term Plan and You initiative. This focuses on the long term mental health and social care needs of the population alongside partner organisations. This scheme blends the level of care and support we provide to individuals within the community. The scheme focuses on implementing an approach which sees organisations co-located, co-producing, and co-delivering care and support to the most vulnerable individuals within the community. We are currently recruiting for two individuals, one on a FTC until June 2024 and one on a FTC until September 2024.

ABOUT THE SERVICE

This role is based within a community setting, the expected outcomes and impact which are thrived to be delivered are:

• To become an integral partner within the blended offer in the community mental health hubs
• Partner with internal and external stakeholders to ensure relevant support and care is in place for the individuals, such as transferring the care to the GP
• Create and maintain a clear pathway to independence, as individuals transition from secondary mental health services to the primary care

KEY RESPONSIBILITIES

• Work alongside partner organisations to build relationships, and ensure all individuals understand their rights and responsibilities, having access to the right tools and resources, to support them back into society and building networks.
• Attend and contribute to blended triage and other meetings with partner organisations to support individuals to achieve their personal goals, in line with their support plan.
• Support individuals on a caseload through one to one meetings and group settings.
• Participate in training in utilising partner organisations client electronic records and any other recording system as deemed necessary for client support needs.
• Build strong and sustainable relationships with internal and external stakeholders and organisations. Such as;Homeless Prevention Officers, Local Landlords, Housing Benefit Teams, Temporary Accommodation and Housing Associations, Hostels, Rehab Facilities, and other relevant organisations.
• Admin duties will vary.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position.

KEY CRITERIA

What we are looking for:

• Experience of working in health, social care, voluntary, and community sector environments
• Experience and knowledge of mental health, and working with mental health services, social care, voluntary and/or recovery focused services
• IT literacy, able to adapt to new platforms

What we would like, but not essential:

• Professional Qualification: NVQ Level 3 in Care or equivalent
