Mental Health Housing Lead (Blended Worker)

Job summary
This role will focus on supporting and empowering individuals to maintain their mental health, also in supporting them to be part of the wider community to help them lead more fulfilling lives. The role will provide pathways, support and appropriate interventions to enable successful resettlements. This role will ensure individuals who are receiving support or require this support, have meaningful conversations about their housing and social care needs.
Main duties of the job
- Work alongside partner organisations to build relationships, and ensure all individuals understand their rights and responsibilities, having access to the right tools and resources, to support them back into society and building networks.
- Attend and contribute to blended triage and other meetings with partner organisations to support individuals to achieve their personal goals, in line with their support plan.
- Support individuals on a caseload through one to one meetings and group settings.
- Participate in training in utilising partner organisations client electronic records and any other recording system as deemed necessary for client support needs.
- Build strong and sustainable relationships with internal and external stakeholders and organisations. Such as;Homeless Prevention Officers, Local Landlords, Housing Benefit Teams, Temporary Accommodation and Housing Associations, Hostels, Rehab Facilities, and other relevant organisations.
- Admin duties will vary.
