Job summary

Rotherham Hospice is seeking a passionate and dedicated Community Outreach Manager to join our team.

This brand-new role is pivotal in expanding access to our palliative and end-of-life services, particularly among minority ethnic groups and patients with rarer, noncancer related life-limiting conditions.

The Community Outreach Manager will work directly with these communities to understand their needs, build relationships, and facilitate connections with the hospice to ensure they receive the support and care they require.

The success of this role will be measured by increased engagement from these patients and groups, ultimately improving access to our services and enhancing the quality of life for those facing life-limiting illnesses.

Main duties of the job

Engagement with Minority Ethnic Communities:

  • Conduct outreach activities to engage with minority ethnic communities within the Rotherham borough.
  • Build trust and rapport with community leaders, organisations, and individuals to better understand their cultural needs and perceptions related to palliative and endof-life care.
  • Develop culturally sensitive approaches and resources to promote awareness of hospice services and address barriers to access. Partnership Development:
  • Collaborate with community organisations, healthcare providers, and other stakeholders to establish partnerships and referral pathways for patients from minority ethnic backgrounds.
  • Identify and leverage existing community resources and networks to enhance support for patients and families facing life-limiting illnesses.

About us

At Rotherham Hospice, we provide exceptional care for all those affected by terminal illness and are the only adult hospice in Rotherham for the people of Rotherham and surrounding villages.

We began offering Day Hospice services in 1994, and have grown over the years to now provide a 14 bed Inpatient Unit alongside our Hospice Community Team who provide care in the community. Our dedicated team of staff and volunteers work together to provide palliative care that incorporates psychological, physical, social and spiritual support to help patients and their families achieve the very best quality of life, both at home and in the Hospice.

Education and Awareness:
  • Working with the Training & Development Manager, deliver educational sessions, workshops, and presentations to community groups, healthcare professionals, and other stakeholders to raise awareness of palliative and end-of-life care options.
  • Provide culturally competent information and support to patients, families, and caregivers regarding advance care planning, symptom management, and other aspects of end-of-life care.
Data Collection and Reporting:
  • Maintain accurate records of outreach activities, engagement outcomes, and service utilisation among minority ethnic communities and patients with rarer conditions.
  • Prepare regular reports and updates for internal stakeholders and funders to demonstrate the impact and effectiveness of the community outreach efforts.
