PURPOSE
As part of the Rotherham Healthwave team, our
Health and Wellbeing Coaches support people from the local community to improve
how they manage their physical, and in turn their mental health. Through working with people both on a one to
one or in a group setting, you will help people to develop the knowledge, skills,
and confidence to change their health-related behaviours, helping to improve their
overall quality of life, health, and wellbeing. These behaviours can include dietary
advice, smoking cessation, and physical activity. As a Health and Wellbeing
Coach you will also be responsible for maintaining up to date and accurate
clinical records. The role requires coaching skills, patience, and empathy to
support people reaching their goals. A person-centred
approach is key to the success of the service, and although the role can
sometimes be challenging it is also very rewarding.
Main duties of the job
1. Manage and maintain a caseload of
clients throughout their chosen lifestyle intervention.
2. Develop individual, evidence based tailored
support plans including recommending treatments based on assessed needs. This
will include individual and group-based interventions, signposting and
referring to other health, community, and voluntary services.
3. Work with clients to set SMART goals using
evidence-based knowledge, and NICE and NHS guidelines.
4. For clients that want to stop
smoking, you will be responsible for giving them advice on stop smoking
medications and vapes and will be responsible for ordering these medications
and ensuring clients know how to use them correctly.
5. To contribute to the monitoring and
evaluation of the service, including the completion of the required data set.
This will include following up clients who do not engage with support and
identifying cohorts with multiple risk factors and most at risk.
6. Ensure that patient related documentation
is entered into electronic health records with high degree of accuracy and
completeness.
7. Work with managers and with IT
support to maintain data quality and make improvements to data collection where
necessary.
8. Assist managers and clinicians in
achieving contracted targets by ensuring data is entered accurately and in a timely
manner.
About us
Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis.
The organisation currently delivers extended access primary care services in Rotherham and a Physio First service in partnership with the local acute and community trust, the largest of its kind currently operating. In addition, the organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce.
