Job summary

“In collaboration with Dementia UK, Connect Healthcare Rotherham (CIC) are looking to expand the current Admiral Nursing Service and recruit five registered nurses (RN, RNMH, RNLD) with a passion for, and experience of, providing relationship-centred dementia care with a strong sense of advocacy and equalities for this client group. This exciting opportunity will see the existing service expand to a team which covers the whole of the Rotherham locality.

Main duties of the job

Admiral Nurses are specialist dementia nurses who work with and support people with dementia and their families who have complex needs.  Under the supervision of the existing Band 7 Admiral Nurse lead, you will provide specialist one-to-one support, guidance, and practical solutions for families, alongside supporting best practice and providing role modelling across Rotherham

About us

You will be employed by Connect Healthcare Rotherham (CIC) with NHS terms and conditions. The team will be based across GP surgeries with some homebased / agile working across Rotherham’s six Primary Care Networks. You will work under the direct management of the Admiral Nurse Clinical Lead and will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse.

In line with the Admiral Nurse Competency framework at an enhanced specialist level, the post holder will:

· Deliver high-quality dementia care through the application of the Admiral Nurse Competency Framework

· Promote, implement, and evaluate evidence-based dementia care, improving outcomes and experiences of those carers/families affected by dementia* registered with a Rotherham GP.

· Support with the delivery of training for health and social care colleagues to improve knowledge and skills in relation to dementia care

· Support the collection and monitoring of evaluation data and performance indicators agreed by Dementia UK and Connect Healthcare to evidence the impact relating to the Admiral Nurse Service

· Liaise and work closely with relevant colleagues and clinical experts in other organisations to establish excellent and seamless pathways across Rotherham Primary Care.

· Work autonomously and manage own time and workload

  • Willingness to work towards continued professional and practice
  • development through Dementia UKs Admiral Nurse Academy,
  • including the level 6 or 7 Admiral Nurse Competency Framework Module
  • Willingness to complete Dementia UKs Pre Admiral Nursing e-learning
  • course prior to starting in post
  • Formal knowledge of dementia/ older people acquired through clinical
  • post-graduate training/ education
  • Registered Nurse (RNM/RNMH/RGN/RNLD)
  • Post-graduate training/ education in dementia/older people

Desirable

  • Additional nursing qualification
  • Mentorship Qualification
