Job Summary:
The purpose of the Pharmacy Technician role is to work as part of a multidisciplinary team of healthcare professionals,
based within GP practice, to support the delivery of safe, effective, and
efficient systems for medicines optimisation, repeat prescribing, reducing
medicine waste and maximising patient outcomes.
Key Responsibilities:
Undertake medicines
reconciliation in line with practice protocols and standard operating
procedures to improve the efficiency and effectiveness of repeat prescriptions

To timely action medication changes in
accordance with Practice Protocols and to update Patients medical records /
prescribing accordingly.

Provide advice and
information to GPs, staff, outside agencies and patients to support the
improvement of prescribing safety, quality, cost-effectiveness, and the
appropriate use of medicines to get the best outcomes for patients, either
face-to-face or over the telephone

To support and coach administrative staff
involved in the preparation of repeat prescriptions to ensure that Practice
Polices and Protocols are adhered to.

To contribute to the development and
implementation of a Medicines Management Policy to support the achievement of
the Practices prescribing Quality and Financial Targets across the cluster

To contribute to the review and development of
Repeat Prescribing Protocols / Standard Operating Procedures to improve the
efficiency and effectiveness of Repeat Prescribing systems.

To provide advice to GPs, staff, outside
agencies and patients face-to-face or over the telephone, with regards to
changes to prescribing that support the improvement of prescribing safety, quality,
and cost effectiveness.

To amend Patients prescribing records in
accordance with Practice Protocols to support safe and cost-effective
prescribing.
-
To take a proactive approach to the safe
handling of prescriptions, including assessment of risk and making
recommendations for improvement.

To contribute to SEAs where medicines
management is a central factor
-
To respond to medication queries from Patients
and Staff in a professional manner, referring to the appropriate GP in
accordance with Practice Protocols.

To advise on the sourcing and safe management
of medicines as appropriate.

To assist in the control of medicine stocks
within the Practice.

To liaise with other
agencies/colleagues/patients with regards to current advice where appropriate

Contribute to the provision of safe
and effective pharmacy projects across the cluster
-
Participate in medication audits and
supporting recall
-
Assist in the monitoring and
management of prescribing budgets and the preparation of evidence-based
resources and information that support cost-effective prescribing.
Administrative
Support:
Other

Taking responsibility for
contributing to all aspects of the day-to-day management of the working
environment including communication, data accuracy and management, keeping
practice areas tidy and helping to achieve a supportive, positive culture
across the practice team.

To support and promote a safe working
environment in accordance with Health & Safety and Infection Control
policies.
Duties may be varied from time to time under the direction of
the Partners / Pharmacist/ Manager, dependent on current and evolving Cluster workload,
and staffing levels.

gather, sensitive information in relation to their health and other matters.

In the performance of the duties outlined in this Job
Description, the post-holder may have access to confidential information
relating to patients and their carers, Practice staff and other healthcare
workers. They may also have access to information relating to the Practice as a
business organisation. All such information from any source is to be regarded
as strictly confidential
