Job summary
As our Recovery Navigator, you will be part of the Community Rehabilitation Service working with and supporting men and women. This involves working alongside organisations within the Bristol Mental Health Partnership. The lead partner of this service is Second Step and the role will be based with them at their Bristol office.
As our Recovery Navigator you will focus on providing appropriate, effective interventions and treatments to people with long term mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
Main duties of the job
Build trusting and collaborative relationships with the service users.Co-produce, implement and review recovery plans, crisis plans and other self-management tools e.g. Wellness Recovery Action Plan, in partnership with service users in line with best practice.
Case manage and co-ordinate support, referring and liaising effectively with all professionals, agencies and other parties involved in service users support in order to deliver the support package. Work closely with colleagues within the service and build strong partnership working with other agencies.
Identify, assess, feedback and manage risk to ensure risk is minimised. Work in a safe manner using a positive risk-taking approach, ensuring the policies of Second Step and Missing Link are implemented and fully adhered to, to ensure all safeguarding issues are properly managed.
Participate actively in supervision and reflective practice. Proactively seek advice and support from colleagues and other agencies as relevant.
Promote and facilitate service user involvement and inclusion within the service and the community.
Contribute to project work, development initiatives and implementation of policies and procedures with guidance from the management team.
Maintain hope and optimism for the individual with high expectations for recovery. Use lived experience (if appropriate) to positively support the individuals recovery.
