Job summary

  • An exciting opportunity has arisen within our Supportive Care Services team, if you are an experienced nurse with community experience looking for a new challenge working across Bury supporting the needs of patients, families and professionals this could be the job for you!

  •  You will need experience, knowledge and an understanding of the philosophy and principles of palliative care. Commitment, enthusiasm, excellent communication and interpersonal skills are vital, alongside a flexible and adaptable approach and a passion for providing high standards of care.

  • Key to the role will be the drive to develop your knowledge and skills of caring for patients with specialist palliative needs and to work as part of the wider multiple disciplinary team

  • COVID-19 vaccination remains the best way to protect yourself, your family, your colleagues and of course our patients from the virus when working in healthcare settings. Main duties of the job

  • We are looking for an enthusiastic, motivated individual with experience and knowledge of caring for patients with a palliative diagnosis. The post holder must possess excellent communication and interpersonal skills and have the ability to influence others in the provision of high quality standards of care for patients and their families at all times, through innovative service delivery. This role requires a dynamic, confident nurse who can influence care by demonstrating excellent clinical and motivational skills.

  • The role encompasses the assessment, implementation and evaluation of holistic care to patients with a life limiting condition and a drive to provide the highest quality service for our patients, and their families/carers, through a collaborative multi-disciplinary team approach.

MAIN DUTIES & RESPONSIBILITIES:

  • Act as a point of contact and work in collaboration with patients, families, carers and other health and social care professionals
  • To ensure a timely and appropriate response, prioritising own workload, in accordance with the needs of patients, families and carers.
  • Undertake holistic assessments for patients and families, regardless of care setting.
  • Support early intervention from hospice services, inclusive of appropriate and efficient discharge and admission to and from the Hospice.
  • Engage in sensitive communication with patients and families enabling them to express their personal preferences and hopes, alleviating their fears and anxieties and supporting them to manage their expectations.
  • Communicate with people clearly in a manner and at a level and pace appropriate to their abilities, preferences and beliefs, treating everybody as equals and with respect.
  • Support patients and families to make informed decisions about accessing hospice services by provision of information. To signpost appropriately to appropriate hospices services and other external providers to meet the needs and wishes of patients and families.
  • Act autonomously and demonstrate a high level of decision making in accordance with the needs and wishes of patients and families.
  • Maintain current and confidential patient records utilising electronic systems in line with Bury Hospice policies and Nursing and Midwifery Council guidance.
  • Attendance at daily Microsoft Teams Meetings alongside other hospices services and external members of the multi-disciplinary team across the locality.
  • Attendance at key forums to promote hospice services and maintain effective working relationships.
  • Attend regular team meetings and support other colleagues as needed.
  • Contribute to the ongoing development and review of services in conjunction with the Clinical Leads team, by engaging in clinical audit, data collection and evaluation of the service.
  • Act as a professional role model at all times for patients, families, carers, staff, students and volunteers.
