An exciting
opportunity has arisen at Bury Hospice to join our Outreach Service. We are seeking to recruit four highly motivated
individuals for the position of Hospice Outreach Support Worker. The service currently operates Monday to Friday 9:00am until 5:00pm, the successful candidates will work 30 hours over four days. As the service develops we aim to operate over seven days.
Our
Hospice Outreach Service is embedded into the
heart of the community. We support patients and families in their own home. We provide education, personal, practical, emotional support, and situational respite.
Main duties of the job
FULL JOB DESCRIPTION IS AVAILABLE TO DOWNLOAD
The role will be to assist and support the
delivery of holistic care. This will be in the provision of face to face
support to patients and families in their own home. You will also be required
to support them to gain a better understanding of their changing health needs,
enabling an individualised care experience.
Candidates must:
Be
willing to undertake extensive travel to attend patients in their homes
NVQ
2 or 3 in health and social care, or equivalent experience
Healthcare experience in a Hospice,
Hospital or Community setting.
Effective
communication skills.
Good
organisational and interpersonal
skills.
Ability
to work independently and in a team.
Able to write concise, legible and
accurate care records.
Ability
to plan and manage own time and
tasks.
Have
a specific interest in palliative care.
