Job summary
This role is an exciting opportunity to work as part of the
Newcastle GP Services (NGPS) outreach team. NGPS are working in collaboration
with Newcastle University to support the supervision of students, and
deliver health interventions and health promotion in the community. This new service
will run 4 days a week for an initial 6 months, with the potential to extend
subject to funding. We are looking to recruit a team of nurses who would be interested in job sharing this role, 1-2 days a week. Outreach clinics will be delivered from a combination of
community based sites, including an outreach vehicle. Working to reduce health
inequalities is at the core of this service, and Core20PLUS5 will be used as a framework

This service will initially deliver low level health
interventions, such as NHS Health Checks, AF AliveCor, immunisations, BP,
smoking cessation, health promotion and signposting. Services will be diversified
and expanded in line with patient and practice need.
We are looking for an experienced Nurse with a background in
General Practice, who is passionate about Primary Care and reducing health inequalities.
This role will involve direct supervision and training of students, and
delivery of clinical interventions in an outreach setting.
Main duties of the job
A commitment to high quality clinical care, excellent
communication, and a passion for tackling health inequalities are essential attributes
for the role of Outreach Nurse.
As the Outreach Nurse, you will be responsible for the delivery
of clinics in community settings and supervision and training of pharmacy and
nursing students. You will be the most senior member of staff on site.

The Outreach Nurse role is initially 6 months fixed term from mid September 2023 to mid March 2024. Hours, pay and contracting are negotiable depending on your experience and availability. minor illnesses, ailments and injuries history taking, physical examination,
problem-solving and clinical decision-making, to establish a diagnosis and
management plan
Prioritise and triage the needs of
patients accordingly, making any necessary referrals for investigations
