Job summary

North Gosforth Primary Care Network (PCN) are recruiting for a Frailty Care Coordinator to join our growing team.

The PCNs is made up of 5 neighboring GP practices working together to provide specialist services to patients in the North Gosforth area of Newcastle.

The successful candidate will work with patients across all 5 practices, supporting anyone identified as frail or at risk of frailty.

You will will devise care plans that focus on patients needs and preferences, aiming towards them being able to continue to live independently in their own homes.

It is a new role, and therefore a great opportunity for a motivated, caring individual to help shape the service and provide real benefits to patients wellbeing.

Advertised on a part time basis of up to 25 hours per week, the contract is for an initial fixed 12 month period.

You will work independently in GP practices and in patients homes supporting individuals in managing their health and wellbeing.

An excellent and confident communicator, you will work effectively with (PCN) colleagues, GP practice staff, patients and their families. Your organisation skills mean you will be able to work independently without direct supervision, and respond to situations calmly and professionally.

Main duties of the job

The role requires integrated working within the PCN. The post holder will need to be able to work independently in a range of different environments including the GP practices and within patients own homes. This role will work with patient to establish what matters to the individual rather than what is the matter and then devise care plans to meet the persons needs. They will work alongside our nursing teams to ensure housebound patients have good access to annual reviews. They will support our social prescribing team, helping with applications for things such as disabled badges and stair lifts, supporting elderly patients to live independently for as long as possible.

Newcastle GP Services (NGPS) is the GP federation for Newcastle-upon-Tyne. We are a member organisation for 28 out of the 31 general practices in Newcastle and have working arrangements with every practice and primary care network in the city. We support them to provide excellent clinical care and a positive patient experience.

NGPS are a skilled team of non-clinicians and clinicians with significant experience working in healthcare. Our role is to help practices work at scale across Newcastle, providing targeted and effective patient care and innovative methods of working to improve quality and efficiency.

  • NVQ Level 3, Advanced level or equivalent qualifications or working towards
  • Demonstrable commitment to professional and personal development
  • Training in motivational coaching and interviewing or equivalent experience

Experience

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
  • Experience of supporting people, families and care in a related role (including unpaid work)
  • Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
  • Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
