Job summary

The Westhoughton Primary Care Network works collaboratively across Peter House and Stablefold surgeries with the provision of primary care services to the residents within this Bolton GP Federation-hosted PCN.

Main duties of the job

The postholder will provide clinical assessment and treatment for the delivery of a safe, efficient, and reliable clinical service. They will work closely with GPs, nursing, and practice teams. They will form links and develop relationships with GP Service colleagues and those working in the wider healthcare community. The post holder will ensure patients receive optimal care in the correct setting within the healthcare system.

Job description

Job responsibilities

The ANP will work with the multi-disciplinary team to ensure appropriate care is delivered in the right place, at the right time. The post holder will be expected to

Work as ANP in the Westhoughton PCN delivering primary care services to the registered practice populations across the network.

Provide timely clinical care to patients in the form of advice or face-to-face consultation whilst on duty.

To evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.

To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings to relate them to a possible diagnosis.

Ensure all consultations, referrals, and investigations are recorded into the patients electronic medical records.

To refer patients to appropriate care settings, within locally agreed pathways.

To discharge or treat, as appropriate and within the scope of practice.

To prescribe as an independent non-medical prescriber, according to agreed protocols and local prescribing formulary.

To maintain accurate and systematic documentation of patient interactions and clinical care provided, including any adverse events.

To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.

To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services, and secondary care providers according to patient need and appropriate care pathways.

To ensure effective communication so that the patient receives the appropriate level of care, and wherever possible, avoiding unnecessary hospital admission.

To take an active role in maintaining effective communication with patients, carers and relatives which meets their individual needs, acting as advocate appropriately.

To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.

Access and support clinical supervision as required for self, and other members of the team.

To proactively communicate with the team to ensure the smooth running of the PCN.

To be responsible for prioritisation and timeliness of all assigned workload and consultations.

Work with colleagues to maximise effective and efficient delivery of care to patients.

To work within the prescribing formulary, taking account of the appropriate length of prescribed treatment.

To record full consultation details using practice templates and protocols.

Register on the GM Passport to enable the PCN administration team to monitor HR and training requirements.

  • Professional registration (NMC, HCPC etc.)
  • MSc in Advanced Clinical Practice.
  • Evidence of CPD
  • Evidence of teaching or assessing in clinical practice.
  • Mentorship or teaching qualification.
  • Degree or Level 7 equivelent

Experience

Essential

  • Broad range of clinical experience in a variety of settings with significant experience at Band 6 or above.
  • Experience of working with a degree of autonomy and high level clinical decision making and accountability.
  • Experience in taking patient history. Able to undertake assessment and critically analyse to make safe clinical decisions.
  • Experience of multi-disciplinary working
  • Demonstrable competency in wide range of clinical skills and willingness to learn.
  • IT literacy

  • Excellent communication and interpersonal skills including emotional intelligence, in order to deal effectively with complex and sensitive patient information, distressed or aggressive patients, or those with an impaired level of understanding.
