The post-holder will be responsible for the on-going financial management and monitoring of all contractual requirements of the PCN. It is desirable that the post-holder have project management and transformation experience (NHS experience preferred)
With responsibility for developing and supporting the delivery of transformational and digital projects across the PCN, the post holder will require excellent communication and engagement skills, specifically in bringing people together and gaining support and buy-in to change.
Main duties of the job
The post-holder will;
support the Clinical Director and PCN Board in all aspects of the development of the Primary Care Network.
Will have day to day responsibility for the financial flows of the PCN, working with the Clinical Director, the Practice Managers and the Board to ensure good financial management of the PCN Resources
be the Digital Transformation Lead for the PCN, supporting the PCN and member practices in adopting new technology and other innovations to improve access to care and enable PCN staff to work more effectively.
Work closely with practice managers to ensure co-ordination and delivery of the Network DES and IIF
Lead the development of a business case for additional estate to support PCN service provision and any subsequent project implementation
PCN Contract Lead for any sub contract arrangements for delivery Network DES
About us
Primary Care Networks are a key vehicle for delivering many of the commitments in the long-term health of the nation plan and providing a wider range of services to patients. They are providing services such as structured medication reviews, enhanced health care in care homes, supporting early cancer diagnosis and understanding local health needs in order to tackle local inequalities.
They work with their partners to provide a wider range of primary care services to patients, involving a wider set of staff roles than might be feasible in individual practices, for example, first contact physiotherapy, extended access and social prescribing.
Networks are also the footprint for deciding which integrated community-based teams will develop, and community and mental health services will be expected to configure their services around PCN boundaries. These teams will provide services to people with more complex needs, providing proactive and anticipatory care.
