We are seeking an experienced, dynamic,
motivated registered social worker with a passion for delivering the best
possible support to our patients to fill this post. You will be a compassionate and positive professional with sound social
work practice working across our 12 bedded Inpatient Unit and Living Well Centre
for outpatient care. You will be working alongside our Advanced Social Work Practitioner/Senior Social
Worker, you will support medical nursing teams with patient discharges from the
IPU, advise on Mental Capacity Assessments and Deprivation of Liberty
Safeguards Application and Safeguarding concerns. Most importantly, you must be
wholeheartedly committed to and motivated by the strategic priorities and
values of the Hospice of the Good Shepherd.
We welcome applications from those
who wish to work 21 hours across 3 or 4 days per week, including Thursday and Friday. Applications will also be considered for 20 hours as above.
Main duties of the job
The post holder will:
Be required to address the psychosocial,
emotional, social, cultural and spiritual dimensions of patients lives and the
impact of these at the end of life across all services.
Build on established links and work with
external service providers to address the needs of patients known to the
hospice and those important to them.
Support the Advanced Social Work Practitioner
to ensure the hospice is effectively implementing the associated safeguarding
policies and procedures.

with their ill health and palliative care needs.
To support patients and carers to access, identify,
appraise, and interpret complex information to enable them to determine their
own future as part of wider MDT.
To support nursing staff on the safe discharge of
patients.
Provide and maintain knowledge and information about
community service provision to contribute to the discharge planning process for
patients, and act as a resource for information and advice.
To be accountable for a caseload of patients and
carers.
To engage in advance care planning, to support
patients, carers and their families in their decision making at the end of
life.
To use good communication skills to deliver
information (which can be complex or highly emotive) clearly, sensitively, and
unambiguously to patients, families, and professionals either face to face, by
telephone or electronic means.
To provide support and information for the immediate
family following the death of a patient.
To complete home visits as appropriate to support
discharge planning and meet the needs of patients and carers.
To liaise between the Hospice, and relevant care
funding bodies, voluntary and other community agencies. Assessing and
negotiating for community-based services for familys carers on discharge.
To support the Approved Social Work Practitioner and
members of the wider MDT in appropriately applying the guidance of the Mental
Capacity Act 2005 and Care Act 2014 and always working within their frameworks
to safeguard the rights and autonomy of patients.
To complete Mental Capacity Assessments, Best
Interests Decisions and Deprivation of Liberty Safeguards.
To use patient identified goals and an understanding
of their social and psychological needs in order to contribute to the
multi-disciplinary team in order to facilitate safe discharge planning for
patients, to home or nursing home.
To support staff in recognising adult and children
safeguarding concerns and follow Hospice safeguarding policy and process.
To keep up to date on current
legislation, welfare rights/reforms that might influence practice when acting
as an advocate for patients to provide the best possible outcome.
To engage in regular 1-1 meetings with the Advanced
Social Work Practitioner, working towards personal and relevant organisational
objectives.
To support the Advanced Social Work Practitioner to
develop, plan, implement and evaluate formal and informal safeguarding training
and education programmes related to palliative care for all health and social
care professionals.
To work alongside and support Living Well Service
designated volunteers.
To maintain and further develop own professional
knowledge and skills.
Required to maintain registration with Social Work
England by completing continuous professional and personal development.
To develop and maintain relationships with
colleagues from the integrated specialist palliative care services and other
Hospices through their social work networks.
