Job summary

You will be based at our Wiltshire or Dorset Hospice and work across Wiltshire and Dorset in the homes of the children we look after and be expected too: To manage the Social Work provision for all Children/Young people and families supported by Julias House. Plan, establish and develop Social Support and the expansion of existing services for Julias House, in close liaison with the Family Support Services Lead and Strategic Nursing Team.

Main duties of the job

To manage the Social Work provision for all Children/Young people and families supported by Julias House. Plan, establish and develop Social Support and the expansion of existing services for Julias House, in close liaison with the Family Support Services Lead and Strategic Nursing Team. To take a lead role within Safeguarding (Adults and Children) for Julias House and for the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards, providing advice and guidance to colleagues in line with local council policy under the direction of the identified Safeguarding Leads. To ensure that Julias House Safeguarding and MCA policies, procedures and training plans are robust, fit for purpose, reviewed and updated in line with Julias House policy review guidance and national guidelines. To hold a caseload of patients and families with significant social, practical and emotional complex needs, delivering responsive and high quality social work support to patients and families.

Job responsibilities

To manage the Social Work provision for all Children/Young people and families supported by Julias House. Plan, establish and develop Social Support and the expansion of existing services for Julias House, in close liaison with the Family Support Services Lead and Strategic Nursing Team. To take a lead role within Safeguarding (Adults and Children) for Julias House and for the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards, providing advice and guidance to colleagues in line with local council policy under the direction of the identified Safeguarding Leads. To ensure that Julias House Safeguarding and MCA policies, procedures and training plans are robust, fit for purpose, reviewed and updated in line with Julias House policy review guidance and national guidelines. To hold a caseload of patients and families with significant social, practical and emotional complex needs, delivering responsive and high quality social work support to patients and families. To keep up to date with National Agendas and relevant professional developments, analyse their impact on Julias House and formulate plans to address these. Advocating for parents in more complex circumstances such as child protection conferences and court hearings (where appropriate). Work within the Care Quality Commission (CQC) regulations and other relevant national standards; help maintain accurate records of methods, evidence of policies and the monitoring of procedures for inspection. Manage the Social Services budget and resources to ensure that the services are delivered in the most effective way. Assist with the high standard of holistic care (physical, psychological, social and spiritual), provided by the Care Team Members of Julias House Hospice as appropriate Assess the needs of families, plan, develop and continue to deliver a service that provides them with access to the Family Support Services which they require, in line with service criteria. Contribute to audit and research to inform and develop good practice. Develop and implement audit tools to monitor the effectiveness of the service and measure service satisfaction levels within the hospice and wider professional sphere.

Qualifications

Essential

  • Applicants for this role must be a Registered Social Worker Qualified to Degree level with current registration with the Health and Care Professions Council (HCPC) with relevant post-qualification experience at Band 6 or equivalent level.

Experience

Essential

  • Experience or training/qualification of working with children/Young People and their families in Palliative Care and Bereavement. Safeguarding Training to Level 4 or working towards.
