This is an
exciting opportunity to come and join our growing team at Newcastle South PCN.
The Advanced Nurse Practitioner will be responsible for
attending to patients who wish to be seen by a healthcare professional,
diagnosing, and treating patients presenting with minor illness, seeing, and
advising patients in respect of their continuing medical and nursing needs,
making any necessary referrals for investigations in the appropriate manner.
We are looking for someone who has excellent communication
and interpersonal skills and who is flexible to meet the needs of the service
and act as a positive role model. Be a team player yet can work autonomously
at advanced practice level and make expert clinical decisions to support the
development of PCN services and inform practice.
The ACNP in primary care will develop
close, long-term relationships with their patients and work in partnership to achieve optimum health.
They are autonomous in making decisions based on assessment, diagnosis, and interpretation of test results.
They can independently prescribe appropriate medication, evaluate, or refer toother specialists when necessary.

Main duties of the job
The following are the core responsibilities of
the ACNP.
There may be, on
occasion, a requirement to carry out other tasks; this will be dependent upon
factors such as workload and staffing levels.
Clinically examine, assess, diagnose, plan, implement and
evaluate treatment/interventions and care for patients, using expertise and
decision-making skills to inform clinical reasoning approaches when dealing
with differentiated and undifferentiated individual presentations and complex
situations; using information from multiple sources to make appropriate,
evidence-based judgements and/or diagnoses.
Diagnose and manage both acute and chronic conditions,
initiating, evaluating, and modifying a range of interventions which may
include prescribing and reviewing medicines for therapeutic effectiveness
(appropriate to patient needs and in accordance with evidence-based practice
and national protocols within scope of practice), therapies, lifestyle advice
and care into a management plan.
As an independent prescriber, prescribe safe, effective, and
appropriate medication in line with current guidance and legislation.

Newcastle South PCN are a
group of practices working together to focus on local patient care.
Newcastle South PCN is a group six GP practices
serving a patient population of approximately 43,000. We are continuing our journey to develop and
deliver innovative services to meet the needs of our local population.
Working from our main base at Ryecroft Surgery in Newcastle
we support Silverdale Medical Centre, Madeley Medical Practice, Kingsbridge
Medical Practice, Keele Practice, Ashely Practice and Betley Practice.
We are a friendly and dynamic team consisting of Clinical
Pharmacists, Pharmacy Technicians, Care Coordinators and Social Prescribing Link
Workers. We also have the support of a
Mental Health Team and First Contact Physiotherapists.
We work
collaboratively with our local GP Federation who support core General Practice and help sustain its future. The Federation are facilitating and
supporting the recruitment of these new roles within our Primary Care Network.
