Job summary

The post holder will be part of a team delivering the Enhancing Health in Care Homes specification to the 11 care homes covered by our primary care network. The role will be responsible for the assessment, diagnosis, treatment and referral of a diverse range of patients within their level of competence and agreed protocols.

MAIN DUTIES AND RESPONSIBILITIES:
1. Clinical
Assessment upon admission of new patients to care homes
Advanced end of life care planning to include every patient being offered ReSPECT discussions
Support prescribing anticipatory care medications where appropriate with the
support of prescribers within the PCN
Proactive, timely and consistent weekly support to the home both in person and virtually using technology enabled care services
Timely same day response to the in-reach teams where appropriate ie: CRIS, PIRT, IST
Provide direct clinical care to patients using established clinical guidelines for both routine and urgent complaints.
Consult and refer patients to specialists and other health professionals as appropriate and in line with locally agreed protocols
Interview patients, take medical histories, perform physical examinations (once physical health assessment qualification gained), analyse, diagnose and explain medical problems
Recommend and explain appropriate diagnostic tests and treatment.
Formulate differential diagnoses and develop and deliver appropriate treatment and management plans. Request and interpret results of laboratory investigations when necessary.
Instruct and educate patients in preventative health care.
Perform both diagnostic and therapeutic procedures following appropriate training and experience as required.
Conduct telephone/video/ face to face consultations which may involve discussing the result and implications of laboratory investigations with patients along with routine and emergency complaints.
Perform proactive, holistic reviews of patients long term conditions and support development of personalised care plans
Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.
Work collaboratively with other members of the PCN including the clinical pharmacy team, dietitian, social prescriber and GP practice staff to deliver a high quality, joined up service to our care home residents
2. Training / Educational Development
Ensure continuing education, training and development is undertaken to meet clinical governance guidelines for Continuing Professional Development and a Personal Development Plan.
Keep up to date with relevant medical research and evidence-based medical practice by attending continuing education courses and professional meetings, reading journals etc.
To have, or be studying for, physical assessment and prescribing qualifications
Perform a variety of research and analysis tasks associated with improvement of clinical care, medical diagnosis and treatment where appropriate using the following means:
- Audit of clinical practice
- Case based reviews
Research unusual symptoms and treatment options through consultation with General Practitioners,
Attend regular multi-disciplinary meetings organised by the PCN in order to
discuss and learn from recent significant events relating to clinical practice occurring within the PCN.
Attend regular educational meetings organised by the PCN or PCN practices in order to update clinical knowledge, practice policy and guidelines and disseminate other useful information relevant to the provision of adequate healthcare for patients.
Regularly reflect on own practice (and keep a record of learning encounters) in order to identify learning needs and encourage self-directed lifelong learning and continued professional development.
3. Administration
Communicate when necessary with colleagues in the PCN in order to discuss or refer specific patients, plan and co-ordinate activities or exchange information in order to improve the quality of patient care.
Send and receive written information on behalf of the Practices within the PCN regarding matters relating to the physical and social welfare of patients.
Work to deliver together with other members of the Practice and Primary Healthcare Team, the objectives of the GP contract, including the terms of the Quality and Outcomes Framework.
Co-ordinate and attend regular multi-disciplinary meetings in order to discuss the health and social needs of particular patients.
KEY & WORKING RELATIONSHIPS:
The Care Home Practitioner and Coordinator will work within our Newcastle North PCN Care Home Time, which includes a Physician Associate, Clinical Pharmacists, Pharmacy Technician and a Dietician, to embed the personalised care approach across the NNPCN
To work as a flexible member of the PCN providing support to other team members when necessary
To take an active role in the development and embedding of the PCNs culture, values and reputation as providers of high quality services

Training may need to be undertaken outside of normal practice hours, and off site
