Job summary
Provide holistic care and support to children and young people with life threatening conditions and their families. The post holder will be required to provide care across a range of settings in the community, hospital and at the hospice and across localities when required predominately to Babies, Children and Young People (BCYP) with Long Term Ventilation (LTV) needs.
Main duties of the job
- To plan and develop, implement and evaluate a specialist LTV service alongside the EACH LTV team and wider multi-disciplinary team.
- Ensuring that patients receive the highest standard of clinical care, safety and support.
- Providing specialist advice, education/training, assessment and care planning and support to health care professionals within EACH and externally.
- Responsibility for contributing to the ongoing development of the LTV service, research and standards of care within EACH.

Job responsibilities
- To contribute to the delivery of care on shift including end of life care if required.
- Mentor new staff in relation to LTV care.
- The post holder will work in collaboration with other services providing care and support for
the child / young person and family.
- To work as part of a team supporting a ventilator dependent child within the hospice/
community setting and to continue that support independently within the hospice and wider
community.
- To be a designated lead for LTV care.
- Contribute to LTV service and care developments, including policy and procedures, under the
clinical direction of the locality Matrons to ensure provision of holistic care and support to
children with LTV care needs.
- Deliver LTV teaching, training and assessment of competency within EACH and also when
required to in the community via commissioned or charitable agreement.
- Develop and deliver teaching, training and assessment of competency in care team staff
through working alongside them in a range of settings in the community and at the hospices.
- Work collaboratively with other services providing LTV care packages for the child and family
as necessary to ensure continuity of delivery of consistent and safe care across agencies.
- To deliver care on shift as required and ensure that holistic care is family centred, needs led,
evidence-based and meets all statutory requirements.
- To apply clinical decision making and practice at a level that reflects skill and competencies in
relation to LTV practice.
