Your new role:
An exciting opportunity has arisen within our Care Directorate for a Night Coordinator for the Inpatient (IPU-Night Service). The Inpatient Unit support St Christophers patients who live in the London boroughs of Bromley, Lambeth, Southwark, Lewisham, and Croydon who require the specialist support the inpatient unit is able to offer. The unit provides care and support seven days a week, 365 days per year. This opportunity will allow the successful jobholder to join a team of night co-ordinators who support both the inpatient service and the St Christophers community service between the hours of 21.00-07.30.
The successful job holder will be required to carry out the following duties but not limited to:
- The successful jobholder will be supported by the In-patient Night Service Team Lead. The role involves ensuring that all requests for St Christophers overnight community service are appropriately triaged between the hours of 21.00 and 07.30. In addition, the successful jobholder will be responsible for managing the inpatient unit between the hours of 20.45- 08.00.
- Ensuring that there is joined up working between the Inpatient unit, St Christophers Community/outpatients provision and the Single Point of Contact to ensure seamless care for the patients and families who use our services overnight.
- Supporting patients to receive appropriate care by signposting to the community out of hours services, directing and supporting the inpatient staff and also the Marie Curie Rapid response service.
To be successful in the role:
- The post holder will ensure high quality care provision overnight including ensuring the medical team are called appropriately and manage the co-ordination of face to face assessment of patients in the Community whose emergency end of life concerns cannot wait until the morning.
- Promote a learning environment at night
- Act as a positive role model at all teams ensuring high quality, patient /carer centred inpatient care is delivered
- Take the role of Bronze commander in a serious incident in the overnight period
