Community Clinical Nurse Specialist / Clinical Paramedic Specialist (Virtual Ward)
Location: Bromley
Hours: Part Time 22.5 hours per week (Monday to Friday) 12 month fixed term contract
Salary: £49,178 - £55,492 (inclusive of HCA, pro rata)

Are you an experienced community palliative care nurse, or paramedic, looking for an exciting opportunity to join a new initiative within the St Christophers Bromley Community Palliative Care Team?

A team that responds rapidly, is holistic, reflexive, responsive and uses all your clinical skills to provide high quality palliative care through direct care, or through supporting other clinicians using teaching, education and advancing practice
The Bromley Hospital At Home Serviceis a new dynamic, collaborative, integrated model of care that aims to deliver a range of monitoring and interventions in the community in order to help patients avoid hospital admission, enable early discharge from hospital and to remain at home through the use of short term support and intervention. St Christophers is part of this new service (The Hospital At Home Palliative Team), and is looking to recruit a part time CNS/CPS to add to the existing team.
What will you be doing? You will work across organisations to support people to remain in their own homes rather than attending hospital. The Palliative Pathway in this innovative service supports people identified as reaching the end of their life .
