Salary:£23,943 per year. (£28,280 full time equivalent)
Job type:Part-time 26 hours per week. Maternity cover. Fixed term contract. (10 hours leadership & 16 hours patient facing).
Location:The successful candidate will be based at Rother House Medical Centre, but must be willing to travel between the PCN practices if required.Due to the geography and nature of the role a clean UK driving license and full use of car is essential.

The Stratford PCN is seeking to recruit a Health and Wellbeing Coach, to further develop and support patient care across the 4 GP practices that form Stratford.
This is an exciting opportunity for you to join a developing Wellbeing Team within the PCN footprint, but to also work in an integrated way with other PCN team members and also the PCN Primary Health Care Teams to provide an all-encompassing approach to personalised care for our patient population. We are looking for a self-motivated individual who wants to make a difference and to support patients to enable them to have better health outcomes. We can offer you a rewarding role within a growing and dynamic PCN Team.

- Experience of supporting people, their families and carers in a related role (including unpaid work)
- Experience of data collection and providing monitoring information
Desirable
- Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
- Experience of working with the VCSE sector (in a paid or unpaid capacity)
- Experience of partnership/collaborative working and of building relationships across a variety of organisations
Qualifications

- Is enrolled in, undertaking or qualified from appropriate health coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide (79), with the training delivered by a training organisation listed by the Personalised Care Institute (80)
- Demonstrable commitment to professional and personal development
- Adheres to a code of ethics and conduct in line with the NHS England and NHS Improvement Health coaching Implementation and Quality Summary Guide
