Job summary
Location: The successful applicant will be based at Waterside Medical Centre, but must be willing to travel to other practices if required.
Work schedule: Full time. 37.5 hours per week. Wednesday's, Thursdays and Fridays are essential.
Salary:£23,871 £24,600. Dependent on qualifications and experience.

Job responsibilities
Management of medical records and clinical correspondence
- Ensure that patient records are accurately maintained
- Processing of clinical correspondence into our clinical systems
- Ensure that clinical correspondence is received and processed in an accurate and timely manner through the Docman system
- Active document management
Screening and Recalls
- Ensure familiarity with all appointment systems including regular and incidental variations
- Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record
- Proactively contact patients to invite them for cancer screening programmes, immunisations and long term condition reviews
- Contribute towards the achievement of Practice and PCN Targets.
Administrative support
- Provide administrative support to members of the primary health care team.
- Administration of EMIS patient searches to support the coordination of project work across the hub.
- Referrals for further care
- Patient and stakeholder liaison
- Able to build relationships with ARRS staff, service users and allied professionals in member practices; to help identify patients who could be referred to other health care professionals within the PCN to help improve PCN efficiencies.
- Provide coordination and navigation to patients across the PCN
- Help people to manage their needs through answering queries, signposting to available services, and ensuring that people have good quality written or verbal information to help them make choices about their care.
- Support delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management.
- Holistically bring together all of a persons identified care and support needs, and explore options to meet these using all available services and resources.
