Job summary

We are looking to recruit a Lead Clinical Pharmacist within Stratforfd PCN to develop and lead our existing clinical pharmacist team and associated services within the Primary Care Network.

37.5 hours per week

Main duties of the job

As the lead Clinical Pharmacist you will oversee, manage and support the work and development of the Clinical Pharmacy team and Pharmacy technicians.

You will work closely with the PCN Manager and wider primary care teams across Stratford Central PCN to ensure successful outcomes within our DES/ IIF contract.

You will spend a minimum of 70% of your time focussing on the DES/IIF work with the other 30% supporting General Practice with their requirements around other contracts such as QOF.

Job responsibilities

Leadership:

Provide proactive leadership on medicines and prescribing systems to the PCN and multidisciplinary team, patient and carers

Lead on the development and roll out of patient facing services and clinics which improve patients outcomes

Line Manage the Clinical Pharmacy team and Pharmacy technicians, providing structured regular supervisions, appraisals and ensure clinical supervision is available and relevant.

Deliver training, monitoring and guidance to the Pharmacy team and direct and oversee the work of this team and the Pharmacy technicians, ensuring their time is utilised effectively across the PCN.

Working in partnership with the PCN Manger, managing the Pharmaceutical elements of the DES/ IIF ensuring all monitoring is completed, concerns and issues are raised in a timely manner and offer a solution focussed service.

Be the main contact for Practices with Pharmacist related requirements including but not limited to: recruitment, inductions, staffing rotas, issues/ concerns in performance, work load and work focus

Clinical:

Provide pharmaceutical consultations to patients with long term conditions as an integral part of multidisciplinary team

Deliver high quality medication reviews and develop bespoke medicines treatment plans for patients (particularly the elderly, complex patients, and those at high risk of admission)

Provide a high quality prescribing service, supporting the GP team as and when required

Organise and oversee the medicines optimisation system including repeat prescribing and medication reviews system

Improve the safety and quality of prescribing following hospital admissions and attendance

Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, prescribing guidance and other clinical standards

Contribute to the achievement and maintenance of good to outstanding CQC registration status.

Self -development:

Develop yourself and the role through participation in clinical supervision, training and CPD

Manage your time effectively to ensure opportunities for growth

Research and offer suggestions for development and training opportunities

  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Good IT skills.
  • Able to obtain and analyse complex technical information.
