Job summary
We are seeking an experienced and passionate RGN to join our Clinical Services Team, working across Hospice at Home and Wellbeing services as Clinical Services Team Leader.
This is a 4 day position (30hours), working 8:30am-4:30pm, across Monday- Friday, with additional evening and weekend telephone on-call responsibilities (worked from home). These responsibilities are shared fairly amongst the qualified staff and receive a generous additional payment.
Working closely with the Clinical Services Manager, we are looking for a professional who will work collaboratively and have a keen interest in the ongoing development of services, ensuring that we are delivering services that meet the needs of the communities of Rossendale.
Main duties of the job
The Team Leader has responsibility for management of the day-to-day running of services, ensuring they are effective, caring, responsive, safe, and well-led. Providing leadership support, education and line-management to qualified and unqualified staff, facilitation of patient groups and undertaking thorough patient-centred assessment/care planning, working collaboratively with the patient, family, carer(s) and those important to them, and the wider multidisciplinary team.
A commitment to excellent standards, with effective communication skills, experience of providing leadership to a team, supporting a caseload of patients and a sound knowledge of and previous work experience within palliative and end of life care are a must for this pivotal role.
A working background which includes community services would be advantageous.
A Palliative/End of Life Care/Long term conditions qualification is also desirable but not essential, however you must be willing to work towards this in post.

- Manage service caseloads of patients and staff within the Hospice at Home community setting and Wellbeing service (day therapy) ensuring the highest quality care is delivered and evidenced through good management, education, effective communication with patients, carers and the wider multidisciplinary team and audits.
- Provide up to date, professional, credible advice and support to colleagues, demonstrating clinical knowledge of the principles and practice of palliative and end of life care, to support the delivery of high-quality care.
- Ensure the effective holistic assessment, planning, implementation, and evaluation of care for patients and their carers and be responsible for care delivery that meets with the high standards set by CQC and hospice policies and procedures.
- Oversee and manage the daily running of clinical services, reacting effectively to changes to ensure service delivery is maintained.
- Work collaboratively to assist with development and maintenance of clinical services that meet with patient and service user need and national and local ambitions.

- To maintain professional development through academic study pertinent to the role in line with NMC the Code for Professional Standards of Practice and Standards of Proficiency for Registered Nurses
- To provide induction training for new staff and manage the mandatory training compliance for the clinical team.
- Act as the educational and placement link for healthcare students and training Doctors.
- Work collaboratively with the Clinical Services Manager in development of the services, participating in audits and overseeing quality standards throughout the clinical services.
