Job summary

This role will utilise health coaching skills to support people develop the knowledge, skills, and confidence to manage their health and wellbeing whilst increasing their ability to access and utilise community support offers through collaborative working with the Social Prescribing Link Workers and the Care Coordinator within the Primary Care Network (PCN) Health & Wellbeing Team, to ensure individuals can access self-management education, peer support and social prescribing.

Main duties of the job

Health and wellbeing coaches will take an approach that considers the whole person in addressing existing issues and encourages proactive prevention of new and existing illnesses. They will take an approach that is non-judgemental, based on strong communication and negotiation skills, which supports personal choice and positive risk taking, addresses potential consequences and ensures patients understand the accountability of their own decisions, making the patient at the centre of every conversation.

They have a contract with Hinckley & Bosworth Medical Alliance Ltd (HBMA) to provide Health & Wellbeing Coaches to work alongside existing PCN staff employed by HBMA working in our 3 Primary Care Networks across the Hinckley & Bosworth locality.

Job responsibilities

PRINCIPAL ACCOUNTABILITIES

1. To undertake holistic assessments, remotely or in the GP surgery, via telephone or video call or at home if the patient is housebound, and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts including within the individuals’ own homes where appropriate.

2. Coach and motivate individuals through single or multiple sessions to identify their needs, set goals and support them to implement their personalised health and care plan.

3. Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through:

· Providing interventions such as self-managed education and peer support;

· Supporting individuals to establish and attain goals set by the individual based on what is important to them, building on goals that are important to the individual;

· Working with the PCN Health and Wellbeing Team and Local Authority partners to connect them to community based activities which support their health and wellbeing.

4. Provide support to local community groups as part of the PCN Health and Wellbeing Team and work with other health, social care and voluntary sector providers to support the individuals’ health and wellbeing in a holistic manner.

5. Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of individuals’ goals where an MDT is involved.

6. Raise awareness within the PCN of shared decision making and decision support tools and supporting individuals in shared decision making conversations.

7. Work with individuals with lower activation to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

8. Explore and support access to a personal health budget, where appropriate, for their care and support.

9. Utilise existing IT and MDT channels to screen patients with an aim to identify those that would benefit from health coaching.

10. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.

11. To ensure you have an understanding of (appropriate to your role), and comply with, safeguarding procedures to protect the welfare of children, vulnerable adults and other groups and individuals.

12. To comply with PCN member practices’ Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others. Whilst doing this to ensure compliance with associated legislation and regulations.

13. To work flexibly as required by the service and to take part in Ripple Touch, G3 PCN and relevant organisations’ meetings and events to promote, support and celebrate the work of the service and the agencies.

  • 2. An appropriate health coaching or similar qualification or the ability to demonstrate an equivalent level of knowledge and understanding relating to the role. A health, social care, local authority, or voluntary sector background is desirable. Experience in a coaching setting (12 months+).
  • 3. Have good communication and negotiation skills and a firm belief that people have untapped resources within them. Work from a strength-based approach focusing on a persons assets.
  • 4. The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working. Whilst have a confident and professional approach to working with a variety of stakeholders.
  • 5. Excellent record keeping skills and the proven ability to write comprehensive reports for a variety of stakeholders.
  • 6. A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures. Enhanced DBS check will be required.
  • 7. A full, clean driving licence.
