Job summary

We are looking for a dedicated Clinical Coordinator to join our team. The person will play a pivotal role in the delivery of our outstanding patient care services, ensuring smooth coordination and support across 7-day-a-week operation.

Main duties of the job

Our new team member(s) will:

  • Provide comprehensive administrative support, inclusive of managing correspondence, entering data onto EMIS, processing and managing new referrals, taking incoming calls and secretarial/administrative work supporting senior medical team and hospice meetings.
  • Work predominantly with the Hospice@Home team and supporting, as well as across other hospice clinical settings as the need arises.
  • Be the first point of contact for patients/healthcare professionals/carers
  • Be confident and empathetic when speaking with distressed patients/ families/carers/relatives and the bereaved over the phone or face to face.
  • Communicate and liaise with all staff efficiently
  • Prioritise workload effectively
  • Support in the coordination of care for people with a life-limiting illness
  • Be confident liaising with a varied team of healthcare professionals.

About us

East Cheshire Hospice is based in Macclesfield and supports patients and families from the High Peak and East Cheshire areas. We take time to see beyond the patients condition to the whole person and their family.

Job description

Job responsibilities

The post holder will be based in the H@H Office and be part of the Clinical Admin Team You will be part of a team that provides administrative support to all members of the Inpatient Unit (IPU)/ HospiceatHome and Palliative Advice Centre East (PACE) Team here at the Hospice. A degree of flexibility is needed with this post to provide cover during periods of absence/ holidays within the team. The emphasis in this role is team working, sharing of knowledge and experience for the overall benefit of the organisation. The post holder must be able to interact calmly and supportively with very ill patients and relatives and distressed visitors. The confidentiality of information about patients must be upheld at all times. As the post involves access to vulnerable adults a satisfactory Disclosure and Barring Service (DBS) check is required.

GENERAL DUTIES

  • To provide a comprehensive administrative support service to members of the H@H Team. This may include processing and managing new referrals, discharge and correspondence and entering data onto EMIS within specified timescales. Ensure all administrative tasks are followed in an accurate and compliant manner following agreed protocols. To support other admin teams as needed e.g. During periods of absence/ holidays.
  • To prioritise work ensuring that correspondence regarding patients is dealt with appropriately and in accordance with service quality standards and time limits. To receive incoming telephone calls, determining the urgency of the call and use own initiative to deal with it. To develop and promote efficiencies in administration processes to the wider clinical teams. To attend and co-ordinate MDT Meetings as required. To support and guide any departmental volunteers and seek advice regarding their practice where required. COMMUNICATION AND RELATIONSHIP Communicating/liaising with all staff within partner organisations, health care professionals and East Cheshire Hospice staff, maintaining good communication/ referral processes with other health care professionals. Dealing with distressed patients, carers, relatives and the bereaved in person/ via telephone. Act as a welcome to the H@H/ IPU Service for all service users. To provide clear and professional communication at all times. Be able to use all communication methods e.g. faxing, e-mailing, making/receiving telephone calls and typing/processing correspondence efficiently & in a timely manner. Ability to discuss workload and any problems/concerns that may have arisen with line manager. Effectively signpost colleagues within the Hospice or external contacts to other agencies or sources of help as appropriate. To continuously seek to maintain and improve own level of skills and undertake learning and development as appropriate.
