Job summary

We are looking for a Social Prescriber to join our Adults Team, working out of General Practices, managing a caseload of Patients.

Social Prescribing is designed to support people with a wide range of social, emotional or practical needs, and many schemes are focused on improving mental health and physical wellbeing. Those who could benefit from social prescribing schemes include people with mild or long-term mental health problems, people with complex needs, people who are socially isolated and those with multiple long-term conditions who frequently attend either primary or secondary health care.

Ongoing career development and training opportunities will be available. Previous experience in health and social care desirable.

Main duties of the job

Conduct holistic assessments with patients to ascertain their social prescribing needs.

Signpost patients to local services/organisations depending on the individuals goals and preferences.

Supporting patients to identify set and meet achievable goals to improve their physical and emotional wellbeing.

Work with partner organisations to support the delivery of an integrated care plan whereby patients needs are met in a holistic way.

Follow up with patients and provide support on an ongoing basis.

Update patient care plans and clinical records.

Develop relationships with local health and wellbeing service providers

Build a network and knowledge of referral routes to and from service providers.

Ensure information about providers and voluntary groups is shared between the wider clinical team.

Support patients to recognise and change their current lifestyle and to identify how their way of life might affect their health and well-being.

About us

WACA is a GP Federation of 8 Primary Care Practices, providing services for a population of 73,000 patients across Airedale, Wharfedale & Craven in Yorkshire. The Primary Care Network services include: Extended Access GP Triage, Pharmacy, Social Prescribing, Physiotherapy & Mental Health Support.

Previous experience in health and social care would be an advantage but training will be provided.
