Community Staff Nurse (NO SPONSORSHIP)

Job summary

We are seeking to recruit a Registered Nurse to join our community team which includes supporting our new Single Point of Access Service (SPA). The role will be supporting our patients and families, providing both telephone and face to face advice. The aim is to enhance and support palliative and end of life care within our catchment area.

Location Lymington, Hampshire.

Hours 15 per week

Main duties of the job

Your day will involve

  • Working very closely with our clinical and wellbeing teams and external agencies
  • Be the initial line of support for patients and families, providing advice, signposting services, considering advance care planning, and providing support in times of distress.
  • Provide the point of access for other health and social care professionals providing relevant end of life support and advice as well as signposting education opportunities.
