Job summary

NHS Health Checks are a systematic prevention programme that assesses an individuals risk of heart disease, stroke, diabetes and kidney disease, once every five years. It is for people aged between 40 and 74 who have not been previously diagnosed with one of the above conditions, hypertension, or are currently receiving certain medications.

The aim of the PCN Heart Health Checks in Surrey Heath Primary Care Network is to provide a quality service that will help people live longer, healthier lives. The longer term aim is a reduction in incidence or early detection of heart attacks and strokes, type II diabetes, chronic kidney disease and vascular dementia.

As a Health Care Assistant with us, you will play a key role working under supervision and delivering our heart health checks which identifies the persons risk of developing cardiovascular disease and diabetes. This health check focus on preventative care, health promotion and education - all within an environment dedicated to providing respectful, high-quality service to every patient. You will have excellent communication skills and will understand the importance of communicating tailored advice and support.

Main duties of the job

To undertake NHS health checks

Phlebotomy every patient will have a blood test as part of the heart health check

Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately

Accurately measure and record clinical measurements such as height, weight and waist measurements

Ensure timely and accurate recording of clinical information and patient interventions on relevant electronic database systems, e.g. EMIS, and following business continuity plans if the electronic system is unavailable

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Provide and communicate relevant health promotion literature and lifestyle advice to patients

Ensure all clinical rooms are adequately stocked and prepared for each session

Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with IPC policy

Work as an effective and responsible team member, supporting others and exploring mechanisms to develop new ways of working.

Support the PCN with health promotion programmes as appropriate

To carry out any other reasonable duties as requested by a manager to ensure quality of service

We are Surrey Heath Community Providers Limited, which is a federation of 7 GP practices across 10 sites, covering a population of over 97,000 patients across Surrey Heath. We began in 2016 and now employ around 200 members of staff across our clinical and corporate support teams. Since 2019 we have hosted the Primary Care Network in Surrey Heath.
