Job summary

We are looking for an experienced Advanced Clinical Practitioner (ACP) to join our team.

The position available is for 20 hours per week,across 3 days - Tuesday, Wednesday and Friday.

The hourly rate for this role is from £22.00 - £33.00, depending on experience

Main duties of the job

You will support core general practice capacity and work as part of a positive, proactive and collaborative team. You will be experienced in general practice and be able to work autonomously, managing a caseload of booked appointments for on-the-day and urgent care needs, with minimal indirect supervision from the onsite GP under a primary care consultant model. You will need to be skilled in providing urgent care and minor illness management and providing support to patients with long term conditions.

  • Significant post registration experience.
  • Recent primary or community experience.
  • Healthcare-led management of minor illness, minor ailments and injuries
  • Healthcare-led triage.
  • Compiling protocols and clinical guidelines.

  • Audit and Research.
  • Non-Medical Prescribing (NMP) Qualification

Desirable

  • Project management.
  • Working with community development initiatives.

  • Experience as a supervisor / facilitator for clinical supervision

  • Advanced clinical practice skills, management of patients with long-term conditions, urgent and on-the-day needs.

  • Strong clinical leadership.
  • Highly effective communication skills, both written and verbal.
  • Negotiation, resource and conflict management.
  • Change management / Service improvement.

  • Maths and English GCSE's (C and above) or equivalent
  • First level professional registration with appropriate professional body (e.g. HCPC, NMC, GPhC).
  • Relevant health-related degree.
  • Advanced Clinical Practice / MSc or equivalent level of practice evidenced through a recognised credentialing process.
  • Mentor/teaching qualification.
  • Extended/independent prescribing qualification.

  • Care of the elderly specialist qualification.
