Job summary
We are looking for an experienced Advanced Clinical Practitioner (ACP) to join our team.
The position available is for 20 hours per week,across 3 days - Tuesday, Wednesday and Friday.
The hourly rate for this role is from £22.00 - £33.00, depending on experience
Main duties of the job
You will support core general practice capacity and work as part of a positive, proactive and collaborative team. You will be experienced in general practice and be able to work autonomously, managing a caseload of booked appointments for on-the-day and urgent care needs, with minimal indirect supervision from the onsite GP under a primary care consultant model. You will need to be skilled in providing urgent care and minor illness management and providing support to patients with long term conditions.

- Significant post registration experience.
- Recent primary or community experience.
- Healthcare-led management of minor illness, minor ailments and injuries
- Healthcare-led triage.
- Compiling protocols and clinical guidelines.

- Non-Medical Prescribing (NMP) Qualification
Desirable
- Project management.

- Advanced Clinical Practice / MSc or equivalent level of practice evidenced through a recognised credentialing process.
- Mentor/teaching qualification.
- Extended/independent prescribing qualification.

- Care of the elderly specialist qualification.
- Additional qualification in management of a specific long term condition.
