Job summary
An exciting opportunity has arisen for a Diabetes Engagement
Officer to support the delivery of the NHS Diabetes Prevention Programme and
various diabetes projects across Frimley ICS.
The Diabetes Engagement Officer will work closely with
healthcare professionals, patients and local communities to encourage patients
at high risk of type 2 diabetes to access lifestyle interventions to reduce
their risk of developing the condition. You will also take an active role in
quality improvement activities and partnership working.
Given the impact of the COVID-19 pandemic on
peoples health and wellbeing, this role provides a real opportunity to make a
difference to peoples lives.
The post holder will be employed by Surrey Heath Community
Providers but will be required to work collaboratively across the ICS.
Main duties of the job
We
are looking for a highly motivated and enthusiastic person to join us, ideally
with experience within a healthcare setting or with health coaching and
confident to build relationships and communicate effectively with a range of
healthcare professionals, local partners and the public.The post holder will be employed by Surrey Heath Community Providers but will be required to work collaboratively across the ICS.
