Job summary

Surrey Heath Community Providers is looking for a new team member to work collaboratively with the practices in Surrey Heath to deliver care services to children and young people. This is a new role brought to the Primary Care Network to help the practices across Surrey Heath with the winter pressures.This role has a level of autonomy, with an expectation of foundational Leadership skills which will be fundamental to provision of leadership, support and guidance to all peers in planning and implementing the service delivering. This role is to work 3 -4 hours per day from 3 pm (after school hours). The role can be worked in the practices or remote and paediatric experience is not essential.

Main duties of the job

Many health conditions, including respiratory system diseases such as asthma, can be caused or worsened by cold weather. Together with higher incidences of so-called seasonal illnesses including flu, norovirus and RSV this means that our practices often faces much greater pressures in the winter. This role is designed to ensure every child has access to a timely high quality and efficient service that can help promote good health and wellbeing and reduce the impact of illness on the child and parent/carers. To promote good health and expert advice to some defined cohorts within the age 0-5 population. To also promote awareness of immune nave Pandemic born children as a cohort. To promote and support the use of online resources for parents, such as Frimley Healthier together (https://frimley-healthiertogether.nhs.uk/). Support and promote the use of the RAG rating for unwell children in triage and in management and follow up.

We are Surrey Heath Community Partners Limited, which is a federation of 7 GP practices across 10 sites, covering a population of over 97,000 patients across Surrey Heath. We began in 2016 and now employ around 200 members of staff across our head office sites, and our primary care and unplanned care services.
