Job summary

Bexley Health Neighbourhood Care (BHNC) is a GP Federation supporting the 22 Bexley Practices / 4 PCNs who are working collaboratively to enhance the health and wellbeing of Bexley residents, covering over 246,000 patients.

This is an exciting opportunity for someone looking to make an impact on the lives of patients living within Bexley and other South London boroughs, to support with multiple projects provided by BHNC within the preventative services team. Currently based in Erith, but due to relocate offices to Bexley Village within the next month, this team delivers a variety of NHS services including structured diabetes education programmes, the South London Diabetes Book and Learn service (DBL), NHS Health Checks and Stop Smoking service among others. These services support patients to manage their own health and wellbeing, with the aim of preventing hospital admission and to care for patients within the primary care setting.

The role will be full time on a rota basis working 7.5 hours per day between 8am -6pm .Consideration will be given to working part of the week from home

Main duties of the job

This role is required to support the delivery of all of the services listed above and others that may feed into this team in due course. The post holder will be expected to work supporting the head of the preventative services team and the preventative services administrator team leader, in the day to day coordination of the project, from the planning and booking of diabetes structured education courses (trainers and venues) to booking patients into structured education courses and all related administration to both of these services, including post course data requests and working with all other providers in South London. As well as supporting with the administration of the NHS Health Checks and Stop Smoking service, such as arranging clinics and equipment ordering, including assisting with audits and service delivery. We would expect the post holder to be able to manage their own work load and meet the deadlines required by the service.

Experience working in the NHS and an understanding of diabetes would be desirable.

Duties

To manage the incoming calls from patients phoning into the DBL call centre, to book onto the diabetes education course of their choice and to support patients who are unsure what course to book. (South London - DBL)

To manage waiting lists within the DBL platform and contact patients who do not engage with website invites, to book onto a diabetes education course of their choice. (South London - DBL)

Manage DNAs by phoning and rebooking, collecting information as to why patient has DNAd for reporting purposes. (South London - DBL)

Working with Priority Digital Health (PDH) and other diabetes education providers, to ensure that courses are delivered to a capacity to meet demand. (South London - DBL)

Acting as the first point of contact for patients accessing the education service. (South London - DBL)

Supporting patients to onboard them onto virtual courses delivered across South London. (South London - DBL)

Planning and booking of the X-PERT Insulin courses for Kings project.

Coordination of incoming referrals for the X-PERT Education courses that we provide outside of DBL (Kings project) and the booking of patients onto the courses.

Planning X-PERT courses (Bexley) for the year with the team lead, creating a year plan for what courses are to be provided.

Booking training venues and liaising with venues for courses (Bexley).

Booking the X-PERT trainers in to the courses they are to deliver (Bexley).

Uploading the courses for patients to book into onto the DBL platform and monitoring the bookings (Bexley).

Creating registers for the courses and arranging with the trainers for collection (Bexley).

Stock monitoring of X-PERT books and ordering when stock runs low (Bexley).

Posting of X-PERT books out to patients who attend virtually (Bexley).

Sending out the links and MS Teams guides to patients who book on to attend courses virtually (Bexley).

Outcome individual registers where needed, by updating DBL platform. (Bexley)

Manage the diabetes mailbox for incoming queries etc (Bexley)

Manage the DBL mailbox for incoming queries.

Receive post and distribute amongst team where necessary.

Support the leads with projects where required.

Manage the X-PERT post course databases for those who attend. (Bexley)

Send out post data requests via email/post to GP practices for patients who have completed the X-PERT programme (Bexley).

Send out invites to patients for NHS Health Checks. (Bexley)

Report lists back to practices of those patients sent an invite for NHS Health Checks. (Bexley)
