Job summary

South Yorkshires Hospices Barnsley Hospice, Bluebell Wood Childrens Hospice, Rotherham Hospice, St Johns Hospice in Doncaster and St Lukes in Sheffield are working together on a project to examine the feasibility of creating transition pathways for young people with life-limiting illnesses moving from childrens to adult services in the county.

The project, funded following an application to the Childrens and Young Peoples Commissioners in South Yorkshire, will run for 18 months and examine issues across both the statutory and charitable sectors, looking at interactions between services ranging from primary care, community and acute providers, and hospices. Existing pathways for young adults are often unclear and uncertain, and the differences between long-term care and end of life care are complex. There are also barriers including regulatory requirements and resources that can stand in the way of joined-up care. These and many other factors will be considered within the scope of the project.

Main duties of the job

The Service Improvement Lead will be the key Project Manager for leading the delivery of this project over the next 18 months. This is an exciting opportunity to make a difference to young people with Life Limiting Diseases who require Palliative or End of Life care.

St Lukes Hospice in Sheffield will be the lead organisation for the project from the collective hospice perspective, with Jo Lenton (St Lukes Director of Care and Lead for Healthcare Partnerships) acting as the Lead for coordination. The post holder will be employed through St Lukes Hospice and will be accountable to the collaborative task and Finish Group for the delivery of the project.

Principal Job Purpose:

The job-holders principal purpose will be to support the strategic and operational delivery of the development of the Palliative and End of Life Transitional Care Pathway undertaking the planning, monitoring, and reporting of the project, working with a range of stakeholders across South Yorkshire.

The project will be a feasibility study to determine the potential for a workable patient pathway to be found around the issue of transition to palliative and end-of-life services for children and young people in SYB within primary and secondary care including the hospice sector.

Key Responsibilities:

An overview of the job-holders key responsibilities include: -

  1. To manage, lead and take responsibility for the delivery of the project that encompasses the Palliative and End of Life Transitional Care Pathway from project scope to project close including where possible the follow on workplan.
  2. To scope, understand and meet the needs of the project across the organisations and with stakeholders ensuring these are mapped and managed throughout the project lifecycle.
  3. To manage, co-ordinate and direct resources assigned to projects on a day-to-day basis to deliver the project's objectives ensuring optimal value is achieved
  4. To produce all project documentation including detailed project plans/work packages, resource and risk plans, exception reports and end stage reports to enable effective project governance, sponsor and stakeholder oversight.
  5. To develop and refresh communications plans (at key project stages) to enable timely information out and an engaged approach to delivery
  6. To report and present on project delivery to relevant Task and Finish Group and Children & Young Peoples Programme Board and Stakeholder groups
  7. To advise and influence internal and external service users and commissioners at all levels, regarding the project delivery to enable proactive and planned engagement, stakeholder and sponsor support, ownership, and risk mitigation.
  8. To develop effective professional partnerships with stakeholders, service providers and commissioners and liaise proactively to ensure sustained project delivery.
  9. To provide strategic advice to senior managers and partner organisations on project delivery, approach and ensure a subject expert input as required for example through leading workshop delivery
  10. To represent the Task and Finish Group as required at key regional and national events to profile project delivery and share good practice.
  11. To keep fully abreast of developments in the transitional care pathways for End-of-Life Care.
