We are looking to
recruit enthusiastic, experienced and motivated Community Nurses to join our
community team. The successful applicants
will be joining an innovative and forward thinking team, contributing to the
delivery of high standards of clinical care across the community of Sheffield.
Main duties of the job
These posts will deliver
palliative and end of life care as part of the St Lukes multidisciplinary
community services delivering specialist skills in patient centred holistic
assessment, symptom management, and effective clinical and advanced care
planning. The role also includes participating in education and training both
internally and externally to the organisation. The post holder will work under
the direct clinical leadership of advanced practitioner level community nurses
and will be responsible to the Matron Community Team and Active Intervention
Centre.
Applicants for these posts
will be first level registered nurses with significant post-registration
experience with a minimum of one years palliative care experience. You will be
motivated, committed and have a flexible approach with a passion to
progress. Evidence of continuing
professional and personal development at diploma level is essential and you
must be a car driver and owner in order to be considered.

The role of the SLH Band D Community Staff Nurse is to deliver palliative and end-of-life
care as part of St Lukes multidisciplinary community service. The clinician will provide specialist
palliative care, knowledge and expertise to families, health and social care
disciplines to enhance the delivery of holistic palliative care. The Staff
Nurse will report to the Community Manager but be led in development by senior
mentors including the Nurse Consultant. The
role is designed to assess and report the current patient status, according to
IPOS and the St Lukes Complexity Framework, and to initiate and deliver
specialist interventions with support from the Community Nurse in Charge each
day.
The clinician will work to contribute to the
community focused services delivered by St Lukes to combine national and local
priorities for end of life care. They
will assist in the running of Community Clinics at Little Common Lane.
The actions of the successful candidate will help
to achieve St Lukes priorities of care.
These include a reduction in the number of avoidable hospital admissions
and the facilitation of choice regarding the place of care at the end of the
patients life.
This position will require a responsive and
flexible individual with skills which contribute to the wider multi-professional
service. The Staff Nurse will be
motivated to develop essential bedside skills, including therapeutic
communication, to support individuals at home.
This role is developmental with a competency framework to ensure ongoing
learning to enable the clinician to progress towards a senior position within
the team.
