Role Summary
To provide a high-quality Palliative Care Service to patients at the end
of life and to provide care and support for their families and carers. The postholder will have excellent written
and verbal communication skills. They
must have experience working within a nursing and medical team and in the safe
administration of medication. Good I.T
skills, are essential. Experience
working in a hospice or palliative care setting is desirable. The ideal candidate will be a team player,
with Leadership and Management skills. A
flexible approach to working is required.
Key Tasks
Quality
Services
Support the Clinical Services Manager and
provide effective clinical leadership to the immediate clinical team
members
To
assist the Lead Nurse in developing a supportive and cohesive team whose
primary purpose is to maintain and deliver excellent standards of care
To
undertake the role of Nurse in Charge as and when required
The
focus of the role is the day-to-day delivery, of excellence in care
provision
Ensure
the observation and supervision of patients and report any identified
concerns to the Nurse in Charge or Registered Manager in a timely manner
Ensure
that patients are supported to maintain independence, social contact,
privacy and dignity
Identify
realistic goals of care and outcomes with patients, ensuring that the
patient fully participates and agrees with plans of care
Record
explicit care plans which reflect the agreed goals and outcomes, which act
as effective tools for all of the multi-disciplinary team
Measure
the effectiveness of the plan of care in achieving the identified goals
and to revise and update care plans accordingly
Participate
in the induction, mentorship, teaching and support of new staff and
students on placement, acting as a positive role model to all staff
Support,
guide and assist Health Care Assistants in carrying out their role in
caring for patients
Ensure
that support staff are only asked to perform tasks that are within their
area of responsibility and level of competence
Be
aware and knowledgeable about the legislative requirements that govern the
Hospice
Demonstrate
clinical expertise and act as a resource / role model for colleagues
Provide
support and care for team member and colleagues, recognising the potential
impact upon them of their working in an emotionally stressful environment
Ensure
the administration of medicines is in compliance with Hospice policy and
Nursing and Midwifery Council guidelines
Exercise
best practice abiding by the Nursing and Midwifery Council Code of
Professional Conduct
Accept
responsibility for the safe handling and storage of drugs and have
knowledge of the Medicine Act (1968) and the Misuse of Drugs Act (1971).
To act in accordance with the NMC code of
conduct and Halton Haven Hospices policies and procedures.
To work as part of a team to provide support
tailored to the individual medial, social, spiritual and emotional needs
of our patients and their families
Work as part of a team to provide
person-centered care to include the assessment, planning, implementation,
updating and evaluation of care from admission to discharge, including end
of life care.
Policies and Procedures
Understand and comply with the policies and
practices of Halton Haven Hospice.
Undertaking of policy development and review in
line with set schedule of work
Attend statutory training in accordance with
Halton Haven Hospice requirements.
Health and Safety
The post holder must familiarise themselves
with matters relating to health and safety management as they affect them
personally and/or Halton Haven Hospice reporting any potential risks to
life or property immediately in accordance with the Halton Haven Hospice
Health & Safety policy and procedures. Staff must use all equipment
provided to undertake their role safely.
Other
Maintain absolute confidentiality in all areas
of work.
Always behave with complete integrity, respect
and professional dignity ensuring actions enhance the reputation of
themselves and the charity.
Any other
duties commensurate with the grade and post.
Communication and
Working relationships
Ensure a
warm welcome to all visitors to the Hospice demonstrating tact, empathy and awareness
of social and cultural differences when communicating with patients, relatives,
visitors and other professionals.
Maintain
all written and electronic patient records in line with Hospice Policy and
Procedure, ensuring confidentiality an compliance with Care Quality Commission
Regulations and the Data Protection Act.
Ensure all
communication is recorded and disseminated as appropriate.
Report any
incidents, infection control concerns or Health and Safety issues, in
accordance with Hospice Policy and Procedure, to the Senior Nurse as soon as
practicable.
Education and
Training
Undertake
any training that may be necessary to completely carry out the post and to
attend all mandatory training.
Actively
participate in the personal development and education of self and team.
Facilitate
an environment, which will be conclusive to learning.
Undergo
performance revised supervisions and appraisals in line with Hospice Policy and
Procedure.
Maintain
own personal and professional development, demonstrating knowledge and skills
in caring for people at end of life in accordance with the Hospice Model and
evidence-based practice.
To be
responsible for own professional self-regulation and abide by the legal
requirements relating to practice.
Assist Lead
Nurses with facilitation of colleague supervision if designated to do.
Maintain
awareness of research and relevant findings and in conjunction with the multi-disciplinary
team-work towards this being applied to practice.
General
Responsibilities
Work within
the Hospice Policies and Procedures at all times.
Maintain
security required for safety of patients, staff and the general public.
Ensure that
Clinical Audit becomes part of everyday practice.
Ensure that
all staff receive a detailed handover at the start and finish of each shift.
Follow
Hospice procedures for managing positive Infection Vontrol and Health and
Safety practices.
Contribute
to practice changes that may be required arising from audits, complaints or
inspection visits.
Ensure the
effective use of Hospice resources.
Monitor the
use of clinical equipment, reporting the need for servicing and repairs or
requests for new or replacement items where appropriate.
Attend and
participate in meetings and events as may be reasonably required by the
Hospice.
Promote the
development of research skills, both in themselves and their colleagues and
encourage cooperation, be receptive to projects and others.
Work in
partnership with the Lead Nurse to ensure a clear understanding of tasks
delegated and complete in a robust and timely manner.
Ensure
Infection Control is maintained in line with requirements specified in Hospice
Policies and Procedures.
Attend a
minimum of two staff meetings per year
