Main duties of the job

You will care for children aged 0-18 with life-limiting conditions, including a variety of complex health needs, offering sessions of respite care both in the home and eventually in our new childrens centre within our Pembury building for day care sessions. We are looking for enthusiastic, flexible team players with a passion for providing high quality holistic care to children and their families, with life-limiting and life-threatening conditions.

You will be an integral part of the team and assist in its development, working alongside childrens registered nurses and volunteers to help us care for our first caseload of children. You will support and enable children, young people and their families to have access to experiences they may not otherwise be able to have. You will communicate effectively and respectfully, sometimes in difficult circumstances, while maintaining accurate records of care on the electronic care record system.

Job description

Job responsibilities

You will play a vital role in the nursing team, delivering an exceptional level of care to children and young people with life-limiting and life-threatening conditions.

You will be committed to providing compassionate, personalised and holistic care, maintaining the dignity of the child and young person and offering support and care to their families and carers within a variety of settings including the childs home, the childrens centre, on outings or within a school setting.

You will work in partnership with the child/ young person and their family to ensure all care needs are met to ensure a good quality of life.

You will be able to initiate and organise play/ art activities in response to a childs/young person needs, promoting choice, independence and self-esteem.

You will safeguard children and young people who access the service and respond to and escalate concerns appropriately.

You will be supported to attend training that enables you to become competent and confident to carry out your role.

Ensure that you maintain an awareness of the multi-cultural and diverse community we service.

Deliverables / outcomes

Deliver high quality delegated care as agreed with registered childrens nursing staff, as directed by the child and young persons individual care plan, promoting play, choice, comfort and quality of life.

Maintain accurate records in accordance with GDPR and confidentiality policy and procedures.

Be able to respond calmly to practical crisis/emergencies and escalate as appropriate, maintaining the comfort, safety, and dignity of the child or young person in their own home as well as within various community settings.

Undertake duties relating to the care and personal care of children and young people.

Take on care that requires technology and particular techniques as highlighted in individualised care plans.

Administer medicines as an authorised practitioner.

To assist children and young people with their dietary needs including alternative routes such as nasogastric or gastrostomy tube.

Assist children and young people with moving and handling utilising safe techniques and use of equipment.

To engage in the planning and delivery of family events and wellbeing sessions.

To undertake other duties at the request of your line manager that are within the scope and purpose of the job role.

Within the role there is an element of lone working, staff to ensure that they follow the appropriate policies and procedures to ensure they remain safe at all times.

Communications and Teamwork

Actively listen and communicate with the children, young people their families and carers in a sensitive and professional manner. Recognising the need to pass on information when necessary.

Attend regular one to one supervision, team meetings and multidisciplinary meetings, sharing and disseminating information as required.

Working with due diligence

Promote the safeguarding and welfare of children young people and vulnerable adults

Recognise and contribute to delivering a quality service, actively participating in clinical governance practices, audits and reviews

Contribute to the on-going development of the Childrens service across all areas of the Hospice.

To always adhere to standards laid out in the infection control policy and procedure

To complete dynamic risk assessments at the beginning of each shift.

Report and record all incidents and near misses and disseminate lessons learnt to ensure an environment of continuous learning and improvement.

To work within applicable ethical, legal and professional frameworks pertinent to childrens palliative care practices.

Training and Development

Complete training essential to individualised care needs of the children and young people, and core competencies as identified in the childrens training framework

Attend training and study days as identified by your manager in line with organisational needs, aims and objectives.

Be proactive in personal and professional development identifying and seeking areas in which to broaden.

Maintain personal development through the Hospice Personal Development Plan. Continuously updating and improving knowledge and competencies.

Develop an area of interest/ specialism to share with other team members and enhance knowledge and learning
