Hours:Full time, 37.5 hours per week. Includes some weekend working, as per rota. (One Saturday in Five Saturday working hours 9:30 3:30) Normal working hours 8:30 4:30.

Job Share can be considered.

Substance Misuse Nurses will deliver services in line with Welsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation.

Clinical services include substitute prescribing for opiate dependence, harm reduction initiatives including the provision of take home Naloxone; family planning and sexual health screening; Blood Borne Virus testing and referral on to specialist support.

Job description

Job responsibilities

Key duties The Substance Use Nurse will:

Facilitate outreach and community engagement to assess, commence and monitor opiate substitute therapy (OST)

Deliver subcutaneous injection of buprenorphine (Buvidal).

Deliver onsite dispensing and supervision of substitute opiate medication.

Follow Controlled Drug Policies and Procedures.

Provide advice, information, and guidance on the management of substance use to service users engaged with services.

Undertake comprehensive clinical assessments

Agree treatment plans with service users, and review them regularly

Undertake drug screening (urine, fingerprint and oral).

Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols

Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration where appropriate and in partnership with Local Health Board and Public Health Wales colleagues.

Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately.

Provide advice, support and training to GPs, pharmacists, and other professionals where appropriate.

Participate in clinical audits and service reviews.

Facilitate the transfer of service users from secondary to primary care services, and lead with referrals / liaison with Community Mental Health Teams (CMHTs) and other community substance use treatment providers on entering and leaving Kaleidoscope services. Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.

Collate and enter data electronically and contribute to service reports.

Maintain and develop personal expertise and knowledge by reading, networking and further training to develop professional competence and to contribute to the development of competence within the team.

Help with the induction and orientation of new staff, volunteers, and students.

Where relevant, develop and maintain the competencies needed to undertake the role of Substance Use Nurse

Ensure professional development by seeking regular supervision and annual appraisals.

Effectively manage own time and prioritise workload, maintaining own administration and accurate personal records regarding travel, diary appointments, TOIL, and expenses incurred.

Be accountable and responsible for own professional practice, operating in line with professional code of conduct and within organisational policy and procedures. Service Development Ensure a high-quality service is delivered in line with the service specification and regional delivery plans.

Contribute to the delivery of internal teaching and training programmes.

Work collaboratively with colleagues to further develop the service This post has been identified as requiring the post holder to provide evidence of BBV immunisation status in order to carry out the role effectively. The role may require the post hold to undertake Dry Blood Spot Testing/ Substance Use Testing/ Working in Needle Exchange or is a clinical role and therefore Kaleidoscope wishes to ensure that staff are not at risk of infection or transmission of a disease that may affect their own or patient safety. Staff will therefore need to evidence their immunisation history, exemption or non-responder on commencement of employment or at routine intervals at the request of the Organisation.
