Job summary

Substance Misuse Nurses will deliver services in line with Welsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation.

Clinical services include substitute prescribing for opiate dependence, harm reduction initiatives including the provision of take home Naloxone; family planning and sexual health screening; Blood Borne Virus testing and referral on to specialist support.

Job description

Job responsibilities

The Substance Misuse Nurse will:

• Deliver onsite dispensing and supervision of substitute opiate medication.

• Follow Controlled Drug Policies and Procedures.

• Provide advice, information, and guidance on the management of substance misuse to service users engaged with services.

• Undertake comprehensive clinical assessments

• Agree treatment plans with service users, and review them regularly

• Undertake drug screening (urine, fingerprint and oral).

• Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols

• Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration where appropriate and in partnership with Local Health Board and PHW colleagues.

• Deliver subcutaneous injection of buprenorphine (Buvidal).

• Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately. Community Substance Misuse Nurse - South Wales

• Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.

• Collate and enter data electronically and contribute to service reports.

Experience

Essential

  • -Excellent care co-ordination skills, with experience in undertaking comprehensive clinical assessment, care-planning and treatment reviews with people who have diverse and complex needs.
  • -Experience and knowledge of working with substance misusers.
  • -Experience of using electronic case management / clinical notes recording system. E.g. PalBase / PARIS/ DELIUS

Desirable

  • -Experience of treating individuals requiring alcohol detoxification.
  • -Experience of developing and maintaining successful partnerships with GPs, treatment agencies and other healthcare providers.
  • -Experience in undertaking clinical audits.
  • -Experience of delivering outreach and engagement model of treatment and care.
  • -Knowledge of housing options pathways and experience of working with individuals at risk of homelessness.

Qualifications

  • RGN/RMN with valid NMC Pin.

Desirable

  • -One year post registration experience in Substance Misuse
  • -Independent/supplementary nurse prescribing qualification
  • -Training in Anaphylaxis and BBV
