Job summary

Our exciting new role, the Community Services Assistant Manager, will work directly under the line management of the Community Services Manager. Our Community Services cover a wide area, Harrogate and District, Hambleton and Richmondshire. The role is operational and requires proficiency in Microsoft Office, as well as extensive experience of domiciliary or other community care. This role is crucial in ensuring the provision of high-quality care within a multi-disciplinary approach across all our services.

Main duties of the job

Key Responsibilities:

  • Management:You will line manage a team of Bank Health Care Assistants, conduct regular meetings, support training, and assist with staff inductions.
  • Operational:You will oversee patient scheduling and staff rota, process referrals, and work closely with the Palliative Care Team and Coordination Services.
  • Governance:You will assist with clinical incident investigations, auditing, policy review, and ensure compliance with CQC standards.
  • Clinical:You will provide contingencies within the clinical team as a Healthcare Assistant when required.

Job description

Job responsibilities

Job Purpose

To work within the Community Services Team, under the line management of theCommunity Services Manager. To be an effective mentor, manager andimplementer of practice, policy and change for our Healthcare Assistant teams toensure the provision of a high level of care (palliative, end of life and social) within amulti-disciplinary approach across all North Yorkshire Hospice Care services.

The roleis predominantly office/home based, with the ability to flex to patient care asrequired, as part of escalation processes.

Key tasks

Management

1. To line manage a team of Bank Health Care Assistants in an empowering andsupportive way, to enable them to be able to deliver excellent standards of care to our patients.

2. To conduct 8 weekly contact meetings, and yearly appraisals with all bankstaff

3. To support the clinical practice educator in competency checking andtraining of all team members.

4. To support the Community Services Manager with all inductions of new staff

Operational

1. To provide operational oversight of patient and client scheduling

2. To provide operational oversight of staff rota

3. To work with the Hospice Services Team to process all referrals, includingcareplan creation and reviewing patients where needed

4. To meet with the Palliative Care Team and coordination services regularly toensure patients needs are being met, with a joint oversight of around 1,800square miles.

5. To support the Community Services Manager with implementation of newservices and approaches to delivering care.

Governance

1. To provide support with clinical incident investigations, auditing and policyreview

2. To partake in meetings to improve governance or operation, providingpersonal, team and organisational perspectives to promote improvements inthe delivery of care and support to our patients, clients, their loved ones andour team members.

3. To support the organisation in systems and data developments to improveoperational effectiveness within our services as required.

4. To work with the Community Services Manager, Head of Community Servicesand the Registered Manager to ensure that Community Services has in place all systems, policies, procedures and practices to ensure compliance with theframework of standards set by CQC, meeting regularly and keeping up todate with changes in our regulatory requirements.

5. Development of post holder to include being part of the ClinicalManagement On Call team, operating on a 1-in-4 weekly basis
