Job summary
An
exciting opportunity has arisen to work within the Springhill Hospice
Specialist Palliative Care Team covering Heywood, Middleton and Rochdale.
We
are looking to recruit an enthusiastic and motivated individual. The ideal
candidate will have a minimum of five years post-registration experience with
previous experience working as a Specialist Palliative Care Nurse, although we
are willing to consider the right candidate with experience of palliative care
patients in a non-palliative care specialist setting.
The
successful candidate will join a multi-disciplinary team providing specialist
palliative care and end of life care to patients and families.

Main duties of the job
Work
within the policy framework of Springhill Hospice in its provision of
specialist palliative and supportive care, by supporting the team in ensuring
that the clinical aims and objectives of the organisation are met.
The
post holder will be experienced as a specialist palliative care nurse, and have
a role in the co-ordination and day to day management of the service in order
to ensure a consistently high standard of service provision.
Establish
and maintain good working relationships and communicate with members of the
multi-disciplinary team, other healthcare professionals and users of the
service to ensure the delivery of high standards of effective specialist
palliative and supportive care.
Actively
participate in the assessment, planning and delivery of a consistently high
standard of specialist palliative and supportive care. Ability to organise,
prioritise and delegate as appropriate and to manage time effectively.
To
support the Community Service Manager by taking the lead in the use of Adastra/I
Care/Documentation and any new initiatives.
Liaising with nursing staff, in particular Hospice at Home.
Act
in accordance with the Nursing and Midwifery Council Code of Conduct.
Although

This role is based within the Heywood, Middleton and Rochdale.
Springhill
Hospice provides the following palliative care services to the local adult
population who are suffering from the effects of life-limiting and terminal
illnesses:
A 16-bed Inpatient
unit providing 24-hour medical and nursing care.
Hospice at
Home Service - supports people who
want to end their days in their own home by providing the support and symptom
management they need to be able to do so.
A Day
Therapies Centre for up to 16 patients, 4-days-a-week, offering
complementary therapies (e.g. reflexology, aromatherapy and massage);
creative and beauty therapies, social activities and clinical support.
Community
Service - provides specialist
palliative care and support to patients and families in their own homes
A 24-hour Palliative
Care Advice Line for patients, families, carers and health professionals.
Bereavement Support for relatives following loss.
Counselling
Services for patients, carers and
relatives, provided by a qualified therapist.
An Education
Unit - an in-house training facility for hospice staff and local health and
social care professionals, with the main focus of training being on the
Springhill Palliative Care Education Passport' (SPCEP) which ensures that core
palliative care skills are taught to all grades of staff.
