difference to those most in need? An exciting opportunity has arisen for
an experienced and highly motivated Paramedic at North Solihull PCN. With a population of just under 37,000
patients you will join our team in supporting the network's GP practices,
all rated as Good by CQC.
You will be a valued member of our multi-disciplinary team of Clinical
Pharmacists and Pharmacy Technician, Paramedics, Physiotherapists, Health and
Wellbeing Coaches, Social Prescribers, Care Coordinators, and Nurse
Associate. You will work alongside a range of local providers from
primary care, community services, social care and the voluntary sector to offer
more personalised, coordinated health and social care to their local patient
populations.
The role will also require you to visit patients in their own home or
in a care home environment, therefore applicants must have a clean driving
license, along with their own car.
Main duties of the job
You must have a minimum of 5 years post-registration experience to apply for this role in order to meet the mandatory ARR requirements. As a PCN Paramedic you will work as an autonomous and highly skilled clinician within a supportive
multidisciplinary team, delivering high-quality, patient-centred care across
North Solihull PCN practices. You will assess,
diagnose and manage a wide range of undifferentiated acute and chronic
conditions, including minor illness and injury, through face-to-face
consultations, telephone triage and home visits.
The role involves prioritising urgent care needs,
developing and implementing treatment plans, and supporting patients to manage
their health through education and self-care. You will also contribute to long-term
condition management, preventative care, and health promotion, while working
collaboratively with GPs and the wider team to ensure safe, effective and
timely care. In addition, you will
support service development, clinical audits, and quality improvement
initiatives to enhance patient outcomes.

telephone triage, taking clinical responsibility for all patients who have
requested an urgent appointment.

for health care professionals and service users, working in collaboration with
other members of the Multidisciplinary Team.

Paramedic, in the provision of a holistic approach for individuals including
