Job summary
Due to the upcoming retirement of our current Nurse. We are seeking an experienced Practice Nurse to join our friendly and supportive team
The successful candidate will be confident in delivering a wide range of nursing services including chronic disease management, immunisations, cervical screening, and health promotion. Working closely with our multidisciplinary team to ensure the best outcomes for our patients.
Main duties of the job
Provide nursing care and support across a range of services including:
Chronic disease management (e.g. diabetes, asthma, COPD)
Cervical cytology
Childhood and adult immunisations
Health promotion and lifestyle advice
Work collaboratively with GPs, HCAs, and other members of the practice team
Participate in clinical audits and contribute to quality improvement initiatives
Support patients in self-management and preventative care
Participate in Practice Meetings when required.
