Job summary

The successful candidate will work across our established sites (New Addington Group Practice & Thornton Road surgery) including our new Community Diagnostic Centre (CDC) in New Addington, offering exposure to enhanced services and modern models of care delivery.

Main duties of the job

We are seeking an ambitious and clinically confident Advanced Clinical Practitioner (ACP) who is ready to work at the top of their license within a supportive and forward-thinking organisation.

The ACP will manage undifferentiated and undiagnosed presentations, deliver high-quality minor illness clinics (primarily face-to-face, alongside telephone and digital consultations), undertake long-term condition management including QOF reviews, and safely interpret and action investigations & documents.

You will be allocated a named Senior GP Mentor and receive structured clinical supervision, ensuring both professional development and safe autonomous practice.

Job responsibilities

Main duties of the job

Provide autonomous clinical assessment, diagnosis and management of acute and chronic presentations (both face-to-face and telephone)

Deliver minor illness clinics and contribute to urgent care capacity.

Undertake chronic disease reviews and population health initiatives (including QOF).

Initiate, interpret and action investigations including blood tests, ECGs and imaging.

Participate in digital triage and workflow management via our online consultation platform.

Conduct home visits and care home reviews where clinically appropriate.

Collaborate within a multidisciplinary team including GPs, pharmacists, nurses and allied health professionals.

Directly refer to secondary care and liaise with external providers.

Contribute to quality improvement projects and service redesign initiatives.

Maintain high-quality documentation and adhere to governance standards.

Key Responsibilities

The post holders main responsibility will be to work within the clinical team to provide high quality patient care.

The post holder will provide clinical assessment and management for patients of all ages (depending on their competencies and experience) with undifferentiated and undiagnosed problems seeking a primary health care service, including minor illness face-to-face consultations, home visits, care home visits and telephone triage and/or consultations.

The post holder must have experience of assessing and treating various illnesses in a primary or urgent care setting.

The post holder may carry out home visits in the locality for patients who are housebound and registered with the practice as required.

The post holder may have responsibility for a care home for which they will be primarily responsible for clinical care, including weekly care home rounds.
