Job summary

The Paramedic will work as part of the multidisciplinary team within the GP surgery, support the delivery of high-quality, patient-centred care. The role involves undertaking clinical assessments, evaluation, and referral. Providing urgent and routine care, managing acute presentations, conducting home visits and supporting chronic disease management. The Paramedic will contribute to reducting GP workload whilst ensuring patients receive safe, effective and timely care.

This role is initially on a 12-month fixed term contract to cover a period of maternity leave.

Full Time (but can be worked with compressed hours) and the salary will be dependent on qualifications and experience.

Main duties of the job

  • The daily triage of patients and prioritise clinical need, referring appropriately to GPs or other healthcare professionals.
  • Undertake urgent and routine home visits as requested by the practice.
  • Weekly ward round and the clinical management of the residents in the aligned care homes referring appropriately to the lead GP for the care home.
  • Provide clinical assessment, diagnosis, and treatment plans for patients presenting with acute illness, injury, or exacerbation of long-term conditions.
  • Prescribe medication (if an Independent Prescriber) or work within Patient Group Directives (PGDs and local pathways).
  • Support the management of patients with chronic diseases, promoting selfcare and health education.
  • Undertake clinical procedures within the scope of practice (e.g., wound care, ECGs, vaccinations if trained). Participate in multi-disciplinary meetings and case reviews.
  • Record consultations, clinical assessments, and outcomes accurately using the clinical system SystmOne
  • Provide advice, support, and signposting to patients, families, and carers.
  • Contribute to safeguarding assessments and reporting.
  • Support the practice in responding to on-the-day urgent demand. Engage in audit, quality improvement projects, and clinical governance activities.

  • Maintain up-to-date knowledge and skills through CPD and mandatory training.

Sharnbrook Surgery is a practice with 6000K+ patients situated in Sharnbrook village in North Bedfordshire. The practice is part of Unity PCN. Sharnbrook Surgery is a dispensing practice.

Our practice team includes: GPs, ANP, Minor Illness Nurses, Practice Nurses, HCA, General Practice Assistants, Dispensary Team and Reception/Admin Team.

Job description

Job responsibilities

  • Experience of working in a GP surgery would be an advantage.
  • Patient triage from Accurx total triage software.
  • Responsible for attending to patients who wish to be seen by a health care professional, diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs.
  • Prioritising and triaging the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
  • Work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
CLINICAL RESPONSIBILITIES
  • Controls admission and discharge to own caseload through:
  • Appropriate comprehensive assessment to include physical, mental, emotional and social health including the use of invasive and non-invasive investigations to establish a diagnosis.
  • Prescription of evidence-based care including medication.

  • Developing with the patient and family when appropriate, an ongoing plan for health with an emphasis on prevention and self-care.
  • Follow-up and evaluation of care.
  • Direct referral to and liaison with consultants and other professional agencies.
  • Making and accepting referrals within the practice team.
  • If qualified to, prescribes according to formulary and competencies.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
  • Support and mange health needs of women presenting for family planning, or sexual health consultation.
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
  • A collaborative, supportive and holistic approach.
  • To keep accurate patient records on computer system.
  • Pathological specimens and investigatory procedure.
  • Undertakes the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs.
  • Checks and processes results received via SystmOne as instructed by the GPs.

Administrative Responsibilities

  • Disseminates learning and information gained to other team members in order to share good practice and inform others about current and future developments/courses.
  • Manages and assess risk within areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  • Participates in the delivery Chronic Disease Management and in achieving QOF Targets.
  • Ensures good communication between you and the team and other members of staff and communicate any changes of ways of working to those affected as appropriate.
  • Attends and contributes to Practice Clinical Governance.
  • Encourages and ensure team working within the practice
  • To participate in practice clinical audits if required. Acts as a resource to other practice members.
  • Uses technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.

  • Formally signed off for competencies of the paramedic FCP/AP road map
