Main duties of the job
Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans.
Provide advanced clinical care, including diagnosis and management of acute and chronic conditions.
Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.
Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated patient care.
Educate patients and their families about health conditions, treatment options, and preventive measures.
Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
Participate in quality improvement initiatives and contribute to the development of clinical guidelines.
Stay current with advancements in medical practices and participate in ongoing professional development.

Provide expert advice to clinicians on prescribing and medicines management.
Support repeat prescription processes and medicine reconciliation post-discharge.
Review and action medication safety alerts and audits in line with current guidance.
Undertake structured medication reviews and deprescribing where appropriate.
Contribute to QOF, IIF, and other clinical quality improvement work.
Provide advice to patients on the safe use of their medicines, side effects, and interactions.
Work collaboratively with community pharmacies and local medicines optimisation teams.
Promote antimicrobial stewardship and best practice prescribing.
Support education and training of staff on prescribing policies and protocols.#

- GPhC registration as a qualified Pharmacist.
- Strong clinical knowledge with experience in primary care, hospital pharmacy, or community setting.
- Independent Prescribing qualification (or working towards).
- Excellent communication, consultation, and organisational skills.

- Strong commitment to continuous professional development.
Desirable
