Main duties of the job
Duties
- Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans.
- Provide advanced clinical care, including diagnosis and management of acute and chronic conditions.
- Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.
- Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated patient care.
- Educate patients and their families about health conditions, treatment options, and preventive measures.
- Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
- Participate in quality improvement initiatives and contribute to the development of clinical guidelines.

Work collaboratively with GPs, nurses, pharmacists, and the wider multidisciplinary team to ensure integrated patient care.

Provide health education, disease prevention advice, and lifestyle support.

Contribute to clinical governance, audits, service improvement, and protocols.

- A relevant degree in nursing or allied health profession with advanced clinical training.
- Registration with the appropriate regulatory body (e.g., NMC or HCPC).
- Significant clinical experience in a relevant healthcare setting.
- Strong assessment, diagnostic, and decision-making skills.
- Excellent interpersonal and communication skills, with the ability to build rapport with patients and colleagues.
- Ability to work independently as well as part of a team.
- Proficiency in using electronic health records and other relevant software applications.
- A commitment to continuous professional development and staying updated on best practices.
Desirable
- Experience using EMIS Web (or other clinical systems).
- Mentorship/supervision qualification.
- Familiarity with QOF and CQC standards.
