Job summary
We are currently seeking a Permanent Part-Time Advanced Nurse Practitioner (Diabetes) to join our team at Latymer Road Surgery. This is a one-day-a-week role, offering a rewarding opportunity for someone passionate about diabetes care and patient support.
Main duties of the job
Managing the care
of diabetic patients, conducting assessments, and developing tailored care
plans.
Educating
patients on diabetes management, including blood glucose monitoring, lifestyle
changes, and medication adherence.
Collaborating with the GP and wider healthcare
team to ensure optimal care for diabetic patients.
Managing
chronic conditions and complications related to diabetes, providing support,
and offering referrals when necessary.
Monitoring patient progress and adjusting care
plans to ensure the best possible outcomes.
Performing advanced nursing assessments and
diagnosing health conditions in diabetic patients.
Prescribing
medications and adjusting treatment plans in accordance with current
guidelines.
Providing
leadership in the management of diabetic care within the practice, including
clinical supervision and mentoring of junior staff.

Job responsibilities
Basing own practice on evidence of good practice in
diabetes and nursing care, leading evidence based improvements and enabling
others to apply them in practice.
Contribute to fortnightly teaching and learning in the
practice.
To work closely with the Integrated Diabetes Service and
other members of the multidisciplinary team including lead consultants, GP's,
nursing colleagues, other health care professionals, social services and
voluntary sector services.
To hold and manage an active caseload.
To undertake patient assessment, develop, implement and
evaluate plans of care for patients with diabetes
To participate in nurse led clinics, outreach and
community clinics and to undertake home visits as required
Undertake clinical procedures and interventions
commensurate with role, including initiating and adjusting insulin doses as
indicated and recommended oral diabetic medication in accordance with national
and local guidelines/practice
Provide telephone support assessment and advice to people
with diabetes on their management in accordance with national and local
guidelines/practice
