Job summary

We are looking at recruiting an enthusiastic and highly motivated Practice Nurse to join our friendly and forward-thinking clinical team. The ideal candidate would be an experienced practice Nurse working in General Practice.

Main duties of the job

We are looking for someone who has experience in chronic disease and case load management to include:

Diabetes

COPD

Asthma

Coronary Heart Disease

Atrial Fibrillation

Anticoagulation Monitoring as well as NOAC and DOAC Therapy

Cervical Cytology

Childhood Immunisations

Travel Immunisations

Vaccination Programme Training in Flu and Covid

Spirometry

Learning Disabilities

About us

We have a fantastic supportive and friendly Clinical and Admin Team. There are three GP partners, two salaried GPs, 1ANP, 2 practice nurses, two HCAs and one practice Pharmacist.

Our clinical system is INPS Vision (changing to Emis in the spring)

We have a list size of approximately 8300 patients

We are part of a fantastic Cluster Network and have great working relationships with the community and secondary care teams, MDT and third sector collaboration working.

Practice Nurse

JOB TITLE: Practice Nurse (Band 6 equivalent)

REPORTS TO: Practice Manager

HOURS: 20 to 37.5 hrs/week (or in accordance with specified rota)

JOB SUMMARY

To provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and any additional roles agreed between the nurse and the doctors as appropriate, having regard to current training.

The Practice has a commitment to excellent patient care, quality improvement and teaching. It is expected that all members of our clinical team are actively committed to supporting these aims and participate in relevant activities when appropriate.

KEY TASKS

To provide clinical nursing service and support to other professionals and to patients of the practice

Work as part of a multi-disciplinary team delivering general medical services to meet the needs of the local population

Providing treatment, screening, chronic disease management and advice within the professional competence of the post holder.

Support and work effectively within the clinical team.

The Practice Nurse may have line management, training and clinical supervision responsibilities for junior nurses and other members of the multidisciplinary team.

All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regard to the various practice protocols.

Experience

Essential

  • Evidence of clinical skills and experience in treatment room consultation
  • Assess complex health & wellbeing needs and develop, monitor and review care plans to meet specific needs
  • Competence in clinical practice associated with the role
  • Problem-solving and decision-making skills
  • Knowledge/experience of clinical systems (e.g. EMIS/SystmOne/Vision etc.)
  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
  • Awareness of the NHS Long Term Plan and Transforming Nursing Plan to develop a general practice nursing role for the future.
  • Competence in clinical practice associated with the role

Desirable

  • Experience of working in a community development context, adult health and social care, learning support or public health/health improvement
  • Substantial experience (post-registration) working in a healthcare setting supervising and/or mentoring junior staff
  • Evidence of professional/clinical knowledge in area supplemented by specialist clinical and/or managerial training
  • Portfolio of CPD with evidence of ability to maintain professional registration
  • Experience of working in primary care
  • Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
  • Experience of GMS Contract and QIF requirements
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations

  • Minimum 2 years experience post registration
  • Post-registration study in topics allied to general practice
  • Chronic disease management qualifications

  • Demonstrate an understanding of audit
  • Understanding of health and safety issues
  • Basic/advanced certificate of life support
  • Undertaken mentorship training
